Hintergrund Die Pandemie durch das Sars-CoV2- Virus (COVID-19) hat die (medizinische) Welt verändert und in zahlreichen Facetten auch Einfluss auf die Gastroenterologie genommen. Ziel der Arbeit Seit Beginn der Pandemie wurde eine große Menge an Informationen über das „severe acute respiratory syndrome coronavirus type 2“ (SARS-CoV-2) und COVID-19 publiziert. Diese Arbeit soll einen Einblick in die Aspekte geben, die für die Gastroenterologie von besonders hoher Relevanz sind. Material und Methoden Es werden Grundlagenarbeiten, Fallserien und Expertenempfehlungen zu gastroenterologischen Aspekten der COVID-19-Pandemie diskutiert. Ergebnisse und Diskussion Gastroenterologische Symptome treten bei COVID-19 zwar häufig auf, haben aber keinen negativen Einfluss auf den Krankheitsverlauf. Ob eine fäkal-orale Übertragung von COVID-19, wie sie durch den Nachweis viraler RNA im Stuhl naheliegt, möglich ist, ist noch nicht abschließend geklärt. Die Fortführung einer immunsuppressiven Therapie bei Patienten mit chronisch-entzündlichen Darmerkrankungen während der COVID-19-Pandemie ist sicher. Eine systemische Steroidtherapie birgt jedoch ein höheres Risiko für einen schweren Verlauf der Infektionskrankheit. Eine Impfung gegen COVID-19 ist auch bei Patienten unter immunsuppressiver Therapie sicher und effektiv. Die COVID-Pandemie hat sektorübergreifend signifikante Auswirkungen auf Patienten- und Versorgungsstrukturen, nicht nur in der Gastroenterologie. Neben den Herausforderungen für die Aufrechterhaltung einer guten medizinischen Versorgung aller Patienten liegen zukünftige Herausforderungen auch darin, die Tätigkeit im Gesundheitssystem durch strukturelle, inhaltliche und psychologische Unterstützung der Akteure attraktiv zu halten und auch unter den Umständen einer Pandemie eine qualitativ hochwertige Ausbildung im pflegerischen und ärztlichen Bereich sicherzustellen.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that caused the COVID-19 pandemic still able to infect the population in many countries. The Omicron strain is the most mutated variant of SARS-CoV-2. The high transmissibility of the strain and the ability to evade immunity require a priority study of its properties in order to quickly create effective means of preventing it. The present work is devoted to the study of in silico interaction of piRNAs with the genome of the SARS-CoV-2 (gRNA) in order to identify endogenous piRNAs and propose synthetic piRNAs with high antiviral activity for drug development. The studies were carried out using proven bioinformatic methods of interaction of the entire SARS-CoV-2 genome with more than eight million piRNAs. Binding sites (BSs) of piRNAs in the 5'UTR were located with overlapping nucleotide sequences called clusters of BSs. Several clusters of BSs were found in the nsp3, nsp7, RNA-dependent RNA polymerase, endoRNAse, S surface glycoprotein, ORF7a and nucleocapsid. 16 synthetic piRNAs have been proposed that interact with gRNA with free binding energy from -170 kJ/mol to -175 kJ/mol, which can be used to create drugs that suppress the reproduction of SARS-CoV-2.

In addition to typical fever and respiratory symptoms, many patients with COVID-19 suffer from a variety of neurological complications and cancer [ 2 , 3 , 4 , 5 ]. The current research aimed to examine the in silico interaction between PIWI-interacting RNAs (piRNAs) and the SARS-CoV-2 genome (gRNA) to identify endogenous piRNAs and propose synthetic piRNAs with strong antiviral activity for drug development. This study used validated bioinformatic approaches regarding the interaction of more than eight million piRNAs with the SARS-CoV-2 genome. The piRNAs’ binding sites (BSs) in the 5′UTR were located with overlapping nucleotide sequences termed clusters of BSs. Several BSs clusters have been found in the nsp3, nsp7, RNA-dependent RNA polymerase, endoRNAse, S surface glycoprotein, ORF7a, and nucleocapsid. Sixteen synthetic piRNAs that interact with gRNA have been proposed with free binding energy ranging from −170 kJ/mol to −175 kJ/mol, which can be used to create drugs that suppress the reproduction of SARS-CoV-2.
