Impact of highly active antiretroviral therapy on the morbidity and mortality in Spanish human immunodeficiency virus-infected children

Abstract: HAART is associated with a marked decline in the progression to AIDS, improved survival in HIV-1-infected children, reduced incidence of infections and hospitalizations and decreased incidence of some organ-specific complications of HIV.

“…In our study, the HAART therapy has allowed for a better cellular immunity due to a significant increase in the count of CD4+ lymphocytes in both age ranges, and such fact was also observed by other authors 4,5,25,26 . This may have been one of the reasons that led the children under HAART, aged bellow 6 years to present lower COM incidence.…”

“…In agreement with our study, it is well established that mortality is generally lower in HIV-infected children on ART compared to those not on ART, with or without TB coinfection. 5,21,22 Our lower mortality figure may be attributed to our HIV programme's policy of prompt screening for TB of all children diagnosed with HIV, and followed by the immediate commencement of ATT prior to early initiation of ART. Our sensitivity analysis examined the worst-case scenario where all LTFU patients were considered to have died.…”

“…Several studies in Europe and the United States have also demonstrated HAART's effectiveness in reducing child mortality (Essajee et al 1999, Teglas et al 2000, Gortmaker et al 2001, van Rossum et al 2001, Goethebuer et al 2009, Kapogiannis et al 2011, Peacock-Villada et al 2011. In a Spanish cohort, HAART decreased the progression of HIV infection, with an estimated reduction of 43% of the risk of developing AIDS after five years (Granados et al 2003).…”
