Impact of Preemptive Granulocyte Infusions During Febrile Neutropenia in Patients Colonized with Carbapenem-Resistant Gram-Negative Bacteria Undergoing Haploidentical Transplantation

Abstract: We prospectively studied the impact of preemptive granulocyte infusions (pGIs) in 69 patients colonized with carbapenem-resistant gram-negative bacteria (CRGNB) undergoing haploidentical hematopoietic cell transplantation (HCT) compared with a previous cohort of 33 patients who received only antimicrobials directed toward CRGNB at the onset of neutropenic fever (non-pGI group). All patients developed neutropenic fever at a median of day +8 (range, À4 to +12) after transplantation. Engraftment kinetics were sim… Show more

“…All patients engrafted neutrophils at a median of 15 days (range, 14‐17) and platelets at a median of 13 days (range, 12‐15 days). All had received at least one donor granulocyte infusion during the neutropenic period as per the existing protocol 7 . None were administered filgrastim.…”

“…As a part of prevention of CRGNB-related sepsis and mortality, pre-emptive granulocyte infusions (pGI) preferably from family members, were administered within 48 hours of the onset of fever if the absolute neutrophil count (ANC) was < 0.5 x 10 9 /l, and on alternate days until the subsidence of fever for greater than 48 hours or neutrophil engraftment as previously described. 8 ABO blood group matched donors received granulocyte colony-stimulating factor (G-CSF) and dexamethasone 8 mg orally, 18-24 hours 4 hours, respectively, before apheresis. 8 The apheresis was carried out on a continuous flow apheresis machine, Spectra Optia® (SPO, Terumo BCT, Lakewood, CO, USA).…”
