In-hospital outcomes after emergency or prophylactic veno-arterial extracorporeal membrane oxygenation during transcatheter aortic valve implantation: a comprehensive review of the literature

Abstract: Veno-arterial extracorporeal membrane oxygenation (V-A ECMO) has been used to deal with life-threatening complications as well as back-up or active cardiovascular support during high-risk procedures in patients undergoing transcatheter aortic valve implantation (TAVI). PubMed and MEDLINE electronic databases were searched in order to identify studies with emergency or prophylactic V-A ECMO application in association with TAVI procedures. From November 2012 to November 2017, 14 relevant studies were identified … Show more

“…First, the retrospective analysis of a small patient population is prone to bias related to unidentified confounders, and the results should be interpreted with caution. Second, use of PCPS was higher in our study compared to previous reports [28]. The reason for this is that prophylactic PCPS is used proactively for high risk cases of haemodynamic instability in our institution.…”

“…As the interventional techniques in cardiovascular medicine continue to develop and ever more critically-ill patient cohorts undergo treatment, the demand for mechanical circulatory support is increasing dramatically in this field. Its prophylactic and bail-out application for percutaneous mitral valve repair and TAVI has been reported with satisfactory outcomes 4 , 5 . The hemocompatible properties of rotary blood pumps commonly used in MCS have not been adequately described.…”

“…ECMO can be used successfully in patients undergoing TAVI as a prophylactic or rescue manoeuvre. 5 During this procedure there is often a short phase of severe aortic regurgitation which was well tolerated in this group of patients.…”
