In‐line filtration of autologous whole blood

Abstract: Leukodepletion of AWB results in a diminished preoperative deposit of RBCs that is pronounced in units with prolonged filtration. The quality of the latter suggests that it is not justified to discard AWB units with prolonged filtration times. Prolonged filtrations are related to patient characteristics that have yet to be defined.

“…The net benefit of this advantage, however, is unclear because leukoreduction results in a loss of RBCs in the WBC filter of up to 10 percent. 14 In conclusion, leukoreduction of AWB does not affect the infection rate and LOS in elective hip surgery. Furthermore, release of BRMs from disintegrating blood cells during storage is probably not a clinically relevant mechanism of TRIM.…”

“…35,36 The findings of this trial could have immediate impact on the logistics and unfavorable cost-effectiveness of PABD programs. 37 On the other hand, quality variables of LD AWB including posttransfusion recovery are better than those of non-LD AWB, 14,38 allowing longer storage of the blood product. This might render restoration of the patients' Hb before surgery more effective.…”

“…Leukoreduction was one major step in the advancement of the quality of WB. Whereas leukoreduction of WB is not of clinical benefit in autologous transfusion, 23‐25 it might be beneficial in allogeneic transfusion because, in contrast to unmodified WB, 26 RBC survival of LR‐WB is comparable to that of RBCs from state‐of‐the‐art leukoreduced RBCs, 27 and labile coagulation factors are preserved during storage 13 . Thus, WB might be of value in clinical settings where disturbances of plasmatic coagulation are to be anticipated 10‐12 .…”

“…RBC units contained a high number of granulocytes. Concerns about cytokine activation during the manipulation steps cannot be excluded, although it was shown recently, that transfusion of not leucocyte depleted, autologous blood did not lead to increased transfusion reactions [23].…”

Concerns about cytokine activation during the manipulation steps cannot be excluded, although it was shown recently, that transfusion of not leucocyte depleted, autologous blood did not lead to increased transfusion reactions [23].Our primary decision to omit the reinfusion of blood ⁄ saline solution from the tubing set into the collection bag in order to avoid dilution of the recovered RBCs has to be revised, because we observed haemolysis in the recovered RBCs requiring additional plasma depletion and …”
