In vitro and in vivo Effects of a Single Dose of Bupivacaine 5% on Donkey Chondrocytes

Abstract: Single intra-articular (IA) injection of long-acting local anesthetics such as bupivacaine is commonly used clinically for postoperative analgesia, in particular, after arthroscopic surgery. Despite their widespread use, the side effects of IA bupivacaine on joint cartilage as well as hepatotoxic and nephrotoxic effects remain to be elucidated. The aim of this study is to assess the in vitro effect of bupivacaine 5% on donkey chondrocytes at different time points, in addition to the in vivo effects of a single… Show more

“…Finally, in 2021, Hussein et al 47 examined the effects of injecting 5 mL of 5% bupivacaine in the middle carpal joint of 10 donkeys and evaluated the joints via radiography, CT imaging, histological evaluation, biochemical evaluation of serum and synovium, and qPCR evaluation of catabolic marker expression following injection. Despite the high concentration of bupivacaine administered there were no significant pathological changes found through any modality in this study.…”

“…6 In a 2-part in vitro and in vivo donkey model, articular cartilage was exposed to 5% bupivacaine for 30 minutes, and outcomes demonstrated a viability of 97.3% and no differences in radiography, histopathology, or expression of cartilage catabolic marker genes. 7 In another investigation, single bupivacaine exposure reduced the viability and weakened the mechanical properties of engineered neocartilage grafts at 24 hours. 2 However, while the viability of mature cartilage explants was also reduced at 24 hours, the mechanical properties were not affected.…”
