Marx was the first to report osteonecrosis of the jaw due to administration of BPs in 2003. Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is the result of an adverse drug reaction. Moreover, many studies worldwide have reported an association between a range of serious dental diseases and the use of bisphosphonates (BPs). Few studies, however, have evaluated BRONJ based on an abnormal signal detected by MRI in the bone marrow of the mandibular condyle. The aim of this study was to assess changes in the magnetic resonance imaging (MRI) signal from the mandibular condyle that could be due to BRONJ. In particular, we focused on the presence of an abnormal MRI signal emanating from mandibular condyle bone marrow. Twenty-eight patients (11 men, 17 women; 56 temporomandibular joints) with BRONJ were evaluated for jaw pain. The patients, whose mean age was 72.9 ± 9.4 years (range 48 -88 years), underwent MRI examination of the jawbone at our hospital from August 2006 to December 2015 and were included in the study. Overall, 80.0% of the patients diagnosed with BRONJ exhibited an abnormal bone marrow signal in the mandibular condyle on the same side of the face that suffered jaw pain. This abnormal signal was present significantly more frequently on the side of the face with the jaw symptoms than on the side without symptoms. Patients with BRONJ displayed an abnormal MRI signal in the mandibular condyle on the side of the face with jaw symptoms, suggesting that MRI findings could be useful clinically for detecting BRONJ in the mandibular condyle.

RESUMEN:Los bisfosfonatos (BF) son fármacos ampliamente utilizados como estabilizadores óseos en el tratamiento de metástasis óseas, osteoporosis, enfermedad de Paget, entre otras patologías, debido a sus efectos anti-tumorales y a la característica de inhibir la actividad osteoclástica. La osteonecrosis maxilar asociada a BF, hoy en día osteonecrosis maxilar asociada a fármacos (ONMF) es definida como la presencia de hueso expuesto, no-vascularizado y necrótico en la cavidad oral por un periodo mayor a ocho semanas, con una historia positiva de tratamiento con fármacos anti-reabsorción ósea (BP, inhibidores del ligando RANKL) y/o anti-angiogénicos y sin antecedentes de tratamiento con radiación o metás-tasis obvia en los maxilares. La frecuencia de ONMF es incierta. La mandíbula es más frecuentemente afectada por ONMF que el maxilar. Pocos casos de ONMF en el maxilar han sido descritos con un diagnostico de sinusitis maxilar simultáneo. Tres casos con sinusitis maxilar asociada a ONMF son presentados en este trabajo. Todos los pacientes fueron mujeres con una historia positiva de cáncer de mama y tratamiento con bisfosfonatos. Los primeros dos casos, desarrollaron ONMF después de una extracción del tercer molar maxilar. El tercer caso con ONMF en el maxilar, sólo tenía antecedentes de curetaje periodontal. Una tomografía computada fue realizada y mostró compromiso del seno maxilar en todos los pacientes. Modalidades diagnósticas para evaluar la extensión de la necrosis y el compromiso del seno, como también alternativas de tratamiento son descritas en este estudio. Finalmente, una revisión actualizada de la literatura es presentada.PALABRAS CLAVE: bisfosfonatos, osteonecrosis maxilar asociada a fármacos, sinusitis.

Osteonecrosis of the jaws is a clinically significant complication of bisphosphonate (BP) medications. Otherwise, the effects of BPs on oral soft tissue or cells remain unknown. The main objective of the present study was to determine whether the presence of sinus mucosal thickening was significantly related to BP-related osteonecrosis of the jaw (BRONJ). A case-control study was conducted on 32 patients who underwent treatment of BRONJ with conventional radiological investigations (panoramic radiographs) and computed tomography. The results indicated that patients with BRONJ had a 5.57-fold greater probability of presenting sinus mucosal thickening than controls. Although the existence of this thickening was more common in patients with advanced-stage disease or low levels of C-telopeptide-cross-linked type I collagen, no significant difference was observed between cases and controls. While considering the limitations inherent in the design and number of cases analyzed in our study, patients with osteonecrosis of the jaw were found to have a 5.57-fold greater probability of presenting sinus mucosal thickening (>3 mm) than healthy subjects.
