Incidence of Transfusion-Related Adverse Reactions per Patient Reflects the Potential Risk of Transfusion Therapy in Japan

Abstract: The incidence of ARs per transfused patient may reflect better the potential risk of transfusion with blood components, taking into account the characteristics of the transfused patient.

“…The average age of www.ee.usp.br/reeusp Rev Esc Enferm USP · 2018;52:e03331 participants in this study was 44 years, which is similar to that found in a study carried out in northwest India (9) , with an average age of 44.3 years. This is well below that found among Japanese receptors, whose average age was 56.7 years, with 94% of reactions occurring among people aged over 18 (10) . Among the patients in this study who had transfusion reactions, we observed a slight difference with regard to gender: 50.7% men and 49.3% women.…”

“…Among severe reactions, 57.1% (12/21) were allergic, followed by TACO at 23.8% (5/21). Among those of greater morbidity were TRALI at 9.5% (2/21) and AHTR at 4.8% (1/21) (8,10,14) . TRALI, which is a severe reaction, is not frequently reported by services and this leads some authors to suggest the occurrence of underreporting as a result of the lack of preparation or knowledge within these services (16,(22)(23) .…”

“…Other studies also have reported a similar incidence of these non-infectious transfusion reactions [20,21]. Therefore, while commonest type of reactions reported in our study were similar to that reported earlier from India, overall incidence was much higher in our study.…”

“…Therefore, reaction per patient rather than the overall incidence seems to be a better predictor of the risk [21] and thus should be reported in all such studies. As our hospital was a new facility, software implementation was initially patchy and patient demographics could not be captured seamlessly.…”

“…This pediatric rate was approximately threefold higher than the adult rate of approximately two per 1000 transfusions. Much of the published data describing rate and frequency of transfusion reactions in adults were obtained from countrywide hemovigilance data and, therefore, do not specifically segregate pediatric from adult data, although these numbers are often referenced as “adult” data since the majority of hospitalized patients are adults . These data reference a range of approximately 2.5 to 3.3 in 1000 .…”
