ObjectivesThis study aimed to identify the epidemiology, clinical characteristics, aetiology and seasonality of sporadic infectious diarrhoea in adults in Shanghai.SettingThis study was based on a city-wide, active continuous hospital-based diarrhoea surveillance network established by Shanghai Municipal Center for Disease Control and Prevention. There were 22 sentinel hospitals in all 16 districts (9 primary-level hospitals, 6 secondary-level hospitals and 7 tertiary-level hospitals) which were selected using probability proportionate to size sampling method.ParticipantsFrom 1 May 2012 to 31 May 2016, 90 713 patients were included in this study. Among 8797 patients whose stool samples were collected and detected, 4392 patients were male.ResultsThe positive rate was 47.96%. Bacterial and viral infections accounted for 27.19% and 69.07% separately. Norovirus was the most common pathogen (43.10%), followed by rotavirus, Vibrio parahaemolyticus, diarrhoeagenic Escherichia coli (DEC) and Salmonella spp. Patients between 30–44 and 45–59 years were more likely to have infectious diarrhoea and viral diarrhoea. Those aged 30–44 years were the most likely to get infected with V. parahaemolyticus (adjusted OR, aOR vs 60+ years: 2.04, 95% CI 1.47 to 2.78) and norovirus (aOR vs 60+ years: 1.32, 95% CI 1.12 to 1.56). Bacterial (except V. parahaemolyticus) diarrhoea was characterised by fever, abdominal pain and loose stool; while viral diarrhoea was characterised by nausea, vomiting and watery stool. A seasonal distribution of infectious diarrhoea was observed with larger peaks in winter and smaller peaks in summer. Winter peaks were mainly due to norovirus and rotavirus, and summer peaks were due to bacterial infections. An emerging spring peak of norovirus around March was observed in the past 3 years.ConclusionViral infections were predominant, and norovirus played a leading role. A seasonal distribution was observed and an emerging spring peak of norovirus was noted. Our findings highlight the necessity for conducting an active, comprehensive surveillance in adults, to monitor changing dynamics in the epidemiology and aetiology of infectious diarrhoea.

Vibrio parahaemolyticus has emerged as a major foodborne pathogen in China, Japan, Thailand, and other Asian countries. In this study, 72 strains of V. parahaemolyticus were isolated from clinical and environmental samples between 2006 and 2014 in Jiangsu, China. The serotypes and six virulence genes including thermostable direct hemolysin (TDR) and TDR-related hemolysin (TRH) genes were assessed among the isolates. Twenty five serotypes were identified and O3:K6 was one of the dominant serotypes. The genetic diversity was assessed by multilocus sequence typing (MLST) analysis, and 48 sequence types (STs) were found, suggesting this V. parahaemolyticus group is widely dispersed and undergoing rapid evolution. A total of 25 strains of pandemic serotypes such as O3:K6, O5:K17, and O1:KUT were identified. It is worth noting that the pandemic serotypes were not exclusively identified from clinical samples, rather, nine strains were also isolated from environmental samples; and some of these strains harbored several virulence genes, which may render those strains pathogenicity potential. Therefore, the emergence of these “environmental” pandemic V. parahaemolyticus strains may poses a new threat to the public health in China. Furthermore, six novel serotypes and 34 novel STs were identified among the 72 isolates, indicating that V. parahaemolyticus were widely distributed and fast evolving in the environment in Jiangsu, China. The findings of this study provide new insight into the phylogenic relationship between V. parahaemolyticus strains of pandemic serotypes from clinical and environmental sources and enhance the MLST database; and our proposed possible O- and K- antigen evolving paths of V. parahaemolyticus may help understand how the serotypes of this dispersed bacterial population evolve.

Celem pracy była analiza źródeł zakażenia z uwzględnieniem zachowań rodziców oraz ich wiedzy na temat dróg przenoszenia bakterii, przebiegu klinicznego oraz objawów zejściowych salmonellozy odzwierzęcej u dzieci z województwa wielkopolskiego w związku z wystąpieniem w regionie ogniska endemicznego choroby. Analizie retrospektywnej poddano dokumentację 35 pacjentów przebywających na dwóch oddziałach zakaźnych Wielkopolski w okresie od maja 2016 r. do kwietnia 2017 r. Zakażenie Salmonella enterica potwierdzono u dzieci w wieku od 3 miesięcy do 15 lat, w znaczącej większości (33 osoby) wcześniej zdrowych. U 30% pacjentów występował w wywiadzie bezpośredni kontakt (spożycie, zabawa) z surowymi produktami pochodzenia zwierzęcego, a rodzice nie mieli świadomości ryzyka zakażenia Salmonella. W 6 przypadkach choroba miała charakter rodzinny. Zespół uogólnionej reakcji zapalnej obserwowano u 11 pacjentów. U 1 dziecka rozpoznano salmonellozowe zapalenie opon mózgowo-rdzeniowych. Dominującymi objawami zakażenia były luźne stolce, które wystąpiły u wszystkich pacjentów i trwały od 1 do 13 dni przed hospitalizacją. Badania laboratoryjne wszystkich pacjentów wykazywały znaczny wzrost poziomu białka C-reaktywnego, natomiast istotna leukocytoza segmentarna obserwowana była w grupie dzieci do 5. roku życia. Wnioski: Zakażenie pałeczkami Salmonella stanowi nadal istotny problem pediatryczny ze względu na możliwość jego wystąpienia w każdej grupie wiekowej oraz ryzyko poważnych powikłań. Ważnym elementem profilaktyki jest edukacja rodziców na temat dróg przenoszenia choroby.
