“Incidental findings” during TAVI work-up: more than just an inconvenience

Abstract: Incidental findings were associated with a longer time to TAVI procedure, lower chance of eventually receiving TAVI as definitive therapy and a worse overall outcome. Such findings are clearly important and should be taken into account when delivering a contemporary TAVI service.

“…Especially in the old and frail TAVI population, potentially malignant incidental findings are common. [8][9][10][11][12][13] Following CTA, a dedicated team considers the patients' suitability for the procedure. Guidelines recommend denying TAVI if the estimated survival is <1 year.…”

“…Until now the only follow-up studies of incidental findings in TAVI workup include a small number of patients who eventually underwent TAVI. [8][9][10][11] The long-term impact of potentially malignant incidental finding after the procedure remains unknown. We aimed to gain insight in the influence of potentially malignant incidental findings on long-term survival after TAVI and evaluate if the findings are an independent predictor of mortality.…”

“…The significance of IF found on MDCT is currently unclear. The definition of IF ranges between studies from simple colorectal diverticulosis or hernias to potentially malignant cancers . Such findings may reduce the likelihood of receiving TAVI and delay time from MDCT to the TAVI procedure .…”

Such findings may reduce the likelihood of receiving TAVI and delay time from MDCT to the TAVI procedure .
