Increased incidence of apparently life‐threatening events due to supine position

Abstract: Gastro-oesophageal reflux (GOR) has a high prevalence in infancy. The supine position is among numerous aggravating factors. The exact relationship between GOR and apparently life-threatening events (ALTE) is not clear, although it has been repeatedly investigated. In 1992 the worldwide Back to Sleep campaign was implemented, which had a dramatic effect on the incidence of sudden infant death syndrome (SIDS) with a drop of 50%. Although the vast majority of children now sleep on their back, the effect of this … Show more

“…11,12 This change in trend in favor of surgery is a combined result of the favorable outcome of this surgery in a number of series and the emerging evidence of the role of gastroesophageal disease in ALTE. 13,14 In our series, 35 of the 79 infants undergoing LNF had one or more documented episode of ALTE. The indications for surgery in infants are somewhat different from those in older children.…”

“…Maggio et al [40] demonstrated an increase in number of apparent life-threatening events after the Back-to-Sleep guidelines had been implemented, most of which were due to gastroesophageal reflux in the supine position on a noninclined mattress. These results emphasize the need to consider reflux and use caution in making 'blanket' recommendations for all infants.…”

“…5 In a Swiss study, pH study was performed in 75 of the 107 infants in the cohort, and GER diagnosed in 75%. 21 In our cohort, majority of the patients with GER suspected on clinical grounds underwent tests to confirm the same. Three investigative techniques were used-pneumography with pH probe, upper gastrointestinal barium study, and radioisotope milk scan.…”
