Indolent T-cell lymphoproliferative disease of the gastrointestinal tract

Abstract: Key Points Ten cases of an indolent T-cell lymphoproliferative disease of the gastrointestinal tract are reported. It is important to recognize this condition because it can be mistaken for aggressive T-cell lymphoma, which may lead to unnecessary therapy.

“…While mucosal glands were displaced by the infiltrate, epitheliotropism was not identified (Figure 4A-C). Small bowel villous blunting was not identified in the patients reported by Perry et al 98 whereas the presence of villous atrophy had suggested a diagnosis of celiac disease in the other report. 97 The infiltrating cells were αb T cells in all cases studied, with a very low proliferative rate.…”

“…[94][95][96] Two recent studies have reported clonal T-cell proliferations of the GI tract with an indolent clinical course. 97,98 In both reports, the patients had GI symptoms suggesting either inflammatory bowel disease or celiac disease, but without response to a gluten free diet. Endoscopy revealed multiple sites of involvement, including oral cavity, esophagus, stomach, small bowel and colon.…”

“…Various risk factors including genetic and molecular alterations,3, 4, 5 diet,3, 6, 7 and chronic inflammation8, 9, 10 have been linked to the development of both disorders. Still other studies have suggested an association between chronic mucosal inflammation and the progression of IBD to pronounced dysplasia in the GI tract 11, 12, 13, 14…”

“…The infiltrates were nondestructive and composed of small, mature-appearing lymphoid cells that were clonal, CD8-positive and, less commonly, CD4-positive or CD4/CD8-negative T cells. The clinical course is indolent, and none of the patients showed progression of tumor size or the extent of involvement [65].…”
