Infektionsinzidenz von Katheterverfahren zur Regionalanästhesie

Abstract: Infectious complications of catheter-based regional anaesthesia are common. Strict hygienic standards must therefore be complied with. More data are necessary to calculate risk factors. The registry provided can also be used as a benchmark to reduce these rates further.

“…Neuburger et al beobachteten in einer prospektiven Analyse bei 5,3-5,5 % der Anlagen von interskalenären Kathetern Inflammationen und bei 2,0-6,8 % Infektionen [30]. Volk [36]. Neuroaxiale Verfahren zeigten ein größeres Risiko einer Infektion im Vergleich zu peripheren Katheterverfahren (2,7 % vs. 1,3 %).…”

“…In addition, peripheral nerve catheters may be inserted to provide prolonged or continuous analgesia but are associated with drawbacks such as dislocation[ 11 ] or bacterial colonization with subsequent possible catheter infection. [ 12 13 ] Breast cancer surgeries may be done on daycare basis, where perineural catheters are not a viable option. [ 14 ] Thus drugs such as dexmedetomidine, which may prolong the duration of single-shot peripheral nerve block, are of particular interest.…”

“…Signs of regional catheter-associated infections were graded according to the predefined criteria [ 2 , 3 , 8 , 9 ] as mild (reddishness, swelling, and painful palpation, ≥2 positive criteria), intermediate (systemic signs of inflammation, e.g., CRP, leukocytosis, fever, and need for antibiotic treatment; ≥2 criteria must be fulfilled), or severe infection (surgical intervention needed).…”

“…However, severe infectious complications may occur such as meningitis or epidural abscess [ 1 ]. Despite standardised antiseptic precautions, bacterial colonisation rates of catheters and catheter insertion sites are quite high (up to 60%) [ 2 , 3 ]. Some authors suggest that a reduction in catheter colonisation rates might lower the incidence of clinical infections, as has been shown for central venous catheters (CVCs).…”
