Evidence accumulates for associations between hypertensive pregnancy disorders and increased cardiovascular risk later. The main goal of this study was to explore shared biomarkers representing common pathogenic pathways between heart failure with preserved ejection fraction (HFpEF) and pre‐eclampsia where these biomarkers might be potentially eligible for cardiovascular risk stratification in women after hypertensive pregnancy disorders. We sought for blood markers in women with diastolic dysfunction in a first literature search, and through a second search, we investigated whether these same biochemical markers were present in pre‐eclampsia.This systematic review and meta‐analysis presents two subsequent systematic searches in PubMed and EMBASE. Search I yielded 3014 studies on biomarkers discriminating women with HFpEF from female controls, of which 13 studies on 11 biochemical markers were included. Cases had HFpEF, and controls had no heart failure. The second search was for studies discriminating women with pre‐eclampsia from women with non‐hypertensive pregnancies with at least one of the biomarkers found in Search I. Search II yielded 1869 studies, of which 51 studies on seven biomarkers were included in meta‐analyses and 79 studies on 12 biomarkers in systematic review.Eleven biological markers differentiated women with diastolic dysfunction from controls, of which the following 10 markers differentiated women with pre‐eclampsia from controls as well: C‐reactive protein, HDL, insulin, fatty acid‐binding protein 4, brain natriuretic peptide, N terminal pro brain natriuretic peptide, adrenomedullin, mid‐region pro adrenomedullin, cardiac troponin I, and cancer antigen 125.Our study supports the hypothesis that HFpEF in women shares a common pathogenic background with pre‐eclampsia. The biomarkers representing inflammatory state, disturbances in myocardial function/structure, and unfavourable lipid metabolism may possibly be eligible for future prognostic tools.

ProblemPreeclampsia affects 3–17% of pregnancies worldwide and has serious consequences for both the mother and the fetus. As maternal–fetal immune tolerance is bidirectional, fetal immunopathology may play a significant role in the pathogenesis of pregnancy disorders. Nevertheless, the impact of preeclampsia on the fetal immune system is unclear.Method of studyIn this case–control study, we examined the phenotype of innate and adaptive immune cells from the cord blood of 3rd trimester babies born to healthy mothers and compared them to cord blood from 3rd trimester babies born to mothers with symptomatic preeclampsia.ResultsThe ratio of CD56hi CD16− non‐activated/regulatory NK cells to CD56lo CD16+ activated/effector NK cells as well as the proportion of CD4+ T cells was significantly decreased in the cord blood of babies born to preeclamptic mothers. The percentage of FoxP3+ Treg, especially the FoxP3lo populations (resting Treg and cytokine Treg), were significantly reduced. Importantly, this reduction in FoxP3+ Treg affected the ratio of CD8+ effector T cells per FoxP3+ Treg in the cord blood of babies born to preeclamptic mothers.ConclusionThese observations indicate that there are significant fetal immune system derangements during preeclampsia.

INTRODUCTIONHypertensive disorders during pregnancy complicate 7-10% of total pregnancies , out of which 70% are preeclamptic. It increases perinatal mortality by five fold and kills 50,000 women worldwide.1 Preeclampsia is a multisystem disorder characterised by hypertension to the extent of 140/90 mm of Hg or more, proteinuria (≥ 300 mg /day) and oedema induced by pregnancy after 20 th week. Without intervention, preeclampsia progresses to eclampsia.Despite considerable research, the pathophysiology of preeclampsia remains unclear. However, oxidative stress has been attributed to be the causative factor of preeclampsia. 3 Lipid peroxidation is event of oxidative stress . Low density lipoproteins are more susceptible for free radical oxidation leading to generation of Malondialdehyde (MDA). This can be the major factor that induces endothelial dysfunction . 4 Slemons and Begort first observed an association between serum uric acid and the presence of pregnancy induced hypertension (PIH). Stander and Cadden were first to demonstrate a high correlation between the ABSTRACT Background: Hypertensive disorders of human pregnancy, such as preeclampsia, complicates a sizeable percentage of all pregnancies, need its early indication and warning. The aim of present study was to determine the association between lipid peroxidation product, serum malondialdehyde (MDA) and serum uric acid levels in preeclampsia and find out any association between these two parameters in preeclampsia. Methods: Thirty preeclampsia and thirty gestational age matched normotensive pregnant women attending Narayana General Hospital, Nellore were included in the study. Serum MDA levels were measured by Thiobarbituric acid reactive substances(TBARS) method and serum uric acid levels were estimated by automated chemistry analyser using commercial kits. Results: Serum MDA (Mean±SEM 24.4±2.38 vs 7.9± 0.28 nmol/ml, p value < 0.000) and serum uric acid levels (7.2 ± 0.25 vs 3.9 ± 0.14 mg/dl, p value <0.000) were significantly elevated in preeclampsia cases when compared with that of normotensive pregnant women. A weak positive correlation between serum uric acid and serum MDA (r value 0.065, p value 0.734) was noticed in preeclampsia. Conclusion: Serum MDA and uric acid may be included as additional markers for screening and progression of preeclampsia, thereby helpful in effectively treating the condition at an early stage.
