In conclusion, DUT treatment in men with BPE led to significant increases in serum FT, TT and LH levels. Significant clinical improvement of AMS scores after DUT treatment with increased serum FT and TT levels were observed in patients with lower FT levels and moderate-to-poor AMS scores accompanied by poor morning erections at baseline. These results suggest that the increases in endogenous FT and TT levels by DUT might have additional benefits of improvement of aging male-related symptoms, apart from the improvement of urinary symptoms, especially in BPE patients with lower FT levels at baseline and moderate-to-poor aging-related symptoms. Introduction: Patients with benign prostatic hyperplasia are usually treated with 5α-reduced inhibitors (5ARIs) such as finasteride and dutasteride. However, studies on the influence of 5ARIs on sexual function have been controversial. In this study, we evaluated the impact of dutasteride treatment for erectile function in patients with once-negative prostate biopsy and benign prostate hyperplasia. Patients and methods: 81 patients with benign prostate hyperplasia were enrolled in a one-armed prospective study. They were administrated 0.5 mg/day of dutasteride for 12 months. Patient characteristics and changes of International Prostate Symptom Score (IPSS) and International Index of Erectile Function (IIEF)-15 scores at baseline and 12 months after dutasteride administration were examined. Results: The mean ± standard deviation (SD) age of the patients was 69.4 ± 4.9 years and the prostate volume was 56.6 ± 21.3 mL, respectively. The mean ± SD prostate volume and PSA levels were decreased 25.0 and 50.9%, respectively, after 12 months of dutasteride administration. IPSS total, voiding subscore, storage subscore, and quality of life score significantly improved after 12 months of dutasteride administration. No statistically significant change in IIEF-total score from 16.3 ± 13.5 to 18.8 ± 16.0 ( p = 0.14), IIEF-EF score from 5.1 ± 6.9 to 6.4 ± 8.3 ( p = 0.13) were observed. There was no decrease in erectile function severity. Conclusion: Twelve months administration of dutasteride for patients with BPH improved urinary function and did not increase the risk of sexual dysfunction.

Objective: This study aimed to evaluate the effect of dutasteride treatment of benign prostatic hyperplasia on serum steroidal sex hormones and prostate volume (PV). Methods: Thirty patients with benign prostate hyperplasia treated with dutasteride for one year were assessed for changes in PV and serum sex hormones (testosterone (T), dihydotestosterone (DHT), estradiol (E2) and dehydroepiandrosterone sulphate (DHEAS)). PV was measured by ultrasonography. A paired or unpaired t-test and Pearson’s correlation coefficient test were used for statistical analysis. Results: DHT and DHEAS decreased by 38.5% and 20.3%, respectively. On the contrary, T and E2 increased by 34.3% and 9.4%, respectively. PV and prostate-specific antigen decreased by 17.7% and 63.4%, respectively. The rate of decrease of DHT had a negative correlation with PV, and the rate of decrease of DHEAS had positive correlation with PV. Conclusion: DHT did not decrease as expected (previous studies showed a decrease of approximately 90%). However, DHEAS decreased more than a natural decrease. It is possible that the conversion from DHEAS to DHT, via the conversion to T, in the prostate was accelerated by the decrease of DHT, especially in larger prostates. However, this discrepancy cannot be full explained by the conversion from DHEAS to DHT, as mentioned above. Level of evidence: Level 2c
