Influence of Islet Transportation on Pancreatic Islet Allotransplantation in Type 1 Diabetic Patients Within the Swiss-French Gragil Network

Abstract: ITT does not seem to influence the efficiency of pancreatic islet allotransplantation in type 1 diabetic patients. These results emphasize the scope for multicenter networks such as the GRAGIL group.

“…The 'islet consortium' concept offers obvious advantages over establishing a new ICPC at each site. Sharing of donor pancreata and isolated islets between centers may also be of assistance in maximizing utilization of donor pancreata (9,10). In this study, we have evaluated the shipment protocol utilized at our ICPC consisting on the use of GPBs, which…”

“…Shipping protocols of islet preparations are less detailed. In the GRAGIL (Groupe Rhin-Rhone-AlpesGeneve pour la Transplantation d'Ilots de Langerhans) experience, islets loaded in a syringe or infusion bag are transported by ambulance to the remote transplant centers where they are transplanted within hours (9,10). Similarly, the University of California Islet Transplant Consortium reported shipping islets in the infusion bag to the remote centers within hours from isolation or after culture in RPMI medium at 37…”

“…Following the Edmonton protocol, the transplantation of islets can successfully induce a long-term restoration of the insulin production, the normalization of the hemoglobin A1C, and the lowering of hypoglycemia excursions (Kessler et al, 2004;Ryan et al, 2005). Nowadays, islets grafting results in 80% insulin independence 1 year after transplantation.…”

“…Furthermore, high costs are involved in establishing and maintaining a GMP-certified isolation facility. Therefore, multicenter IT networks have been established around a single islet isolation facility [2,3]. An appropriate method of islet transportation must be established to transport isolated islets from the islet isolation facility to the transplant hospitals.…”
