Long-Term Outcomes of Vibroplasty Coupler Implantations to Treat Mixed/Conductive Hearing Loss

Objective: To evaluate the long-term safety and performance of four different vibroplasty couplers (round window, oval window, CliP and Bell coupler) in combination with an active middle ear implant. Methods: This was a multicentre, prospective, long-term study including 5 German hospitals. Thirty adult subjects suffering from conductive or mixed hearing loss were initially enrolled for the study, 24 of these were included in the final analysis with up to 36 months of postsurgical follow-up data. Bone conduction and air conduction were measured pre- and postoperatively to evalu ate safety. Postoperative aided sound field thresholds and Freiburger monosyllable word recognition scores were compared to unaided pre-implantation results to confirm performance. Additional speech tests compared postoperative unaided with aided results. To determine patient satisfaction, an established quality-of-life questionnaire developed for conventional hearing aid usage was administered to all subjects. Results: Mean postoperative bone conduction thresholds remained stable throughout the whole study period. Mean functional gain for all couplers investigated was 38.5 ± 11.4 dB HL (12 months) and 38.8 ± 12.5 dB HL (36 months). Mean word recognition scores at 65 dB SPL increased from 2.9% in the unaided by 64.2% to 67.1% in the aided situation. The mean postoperative speech reception in quiet (or 50% understanding of words in sentences) shows a speech intelligibility improvement at 36 months of 17.8 ± 12.4 dB SPL over the unaided condition. The signal-to-noise ratio (SNR) improved by 5.9 ± 7.2 dB SNR over the unaided condition. High subjective device satisfaction was reflected by the International Inventory for Hearing Aids scored very positively. Conclusion: A significant improvement was seen with all couplers, and audiological performance did not significantly differ between 12 and 36 months after surgery.

<p>Chronic suppurative otitis media occurs in the population as a common disease among people living in low socio-economic status. The associated complaints such as hearing difficulty, irritation, discharge makes it difficult for the people to carry out their daily activities. Therefore, their immediate need for treatment occurs. This review aims at analysing the post-operative outcome of hearing following tympanoplasty in order to overcome their morbidity. The source of this study has been obtained from Pubmed, Google scholar and EMBASE. The material for this review was selected from a reference published between January 1975 to December 2014, with this analysis was made. The pre-op ABG in the studies were all falling in mild to moderate hearing loss according to WHO classification of hearing loss. The patients were grouped in Austin-Kartush criteria, groups A to E. The Austin-Kartush classification group A had 3 studies included. The mean ABG for each were 11.5,8.3 and 13.5. In group B,3studies were included. The mean ABG for each were 11.9,10.9 and 11.1. Group C had 4 studies. The mean ABG were 17.7,15,17.1 and 15.7. Group D had 4 studies included which had the same mean ABG 16.6. Group E had just 1 study of mean ABG 20.1. This review helped to establish that there was significant improvement in hearing post-surgery, in patients with varying degrees of hearing loss.</p>

Techniques and biomaterials for reconstructive middle ear surgery are under steady, ongoing development. At the same time, clinical post-surgery results are evaluated to determine success or failure of the therapy. Routine quality assessment and assurance is of growing importance in the medical field, and therefore also in middle ear surgery. The exact definition and acquisition of outcome parameters is essential for both a comprehensive and detailed quality assurance. These parameters are not the audiological results alone, but also additional individual parameters which influence the postoperative outcome after tympanoplasty. Selection of patients and the preoperative clinical situation, the extent of the ossicular chain destruction, the chosen reconstruction technique and material, the audiometric frequency selection and the observational interval are only some of them. If these parameters are not well documented comparative analyses between different studies are of limited value. The present overview aims to describe, compare and evaluate some of the existing assessment and scoring systems for middle ear surgery. Additionally, new methods for an intraoperative quality assessment in ossiculoplasty and the postoperative evaluation of suboptimal hearing results with imaging techniques are available. In the area of implant development functional elements were integrated in prostheses to enable not only good sound transmission but also compensation of occurring atmospheric pressure changes. In combination with other components for ossicular repair they can be used in a modular manner which, so far show experimentally and clinically promising results.
