Introduction/Objective Malocclusions are one of the most frequent disorders in dentistry, and pose a risk for the onset of caries and periodontal diseases. Fixed orthodontic treatment solves the problem of malocclusions; however, it requires simultaneous cooperation of the patients, parents, and dentists involved. The objective of this study is to examine the effects of fixed orthodontic therapy on the Streptococcus mutans and Lactobacillus spp. bacteria in saliva, the pH value, and buffering capacity of saliva. Methods The research was carried out at the Faculty of Medicine in Foča, Department of Dentistry. The study included 100 respondents, aged 13 to 17 years. The respondents were divided into two groups: the study group (respondents wearing fixed braces) and the control group (respondents not subjected to fixed braces therapy). Saliva samples were taken from the respondents four, 12, and 18 weeks after the start of the orthodontic therapy. The study used the bacteria caries risk test (CRT) and CRT buffer (Ivoclar Vivadent). Results The study showed an increased number of bacteria in saliva of the respondents during all three follow-up periods (χ 2 test, p = 0.001). The largest numbers of the Streptococcus mutans and Lactobacillus spp. bacteria were found in week 12 of the therapy. Saliva pH value and buffering capacity of saliva increased statistically significantly in week 12 of the therapy (χ 2 test, p = 0.001). Conclusion Oral conditions in patients changed during the fixed orthodontic therapy: the number of bacteria increased, the pH value and buffering capacity of saliva changed. It was necessary to use preventive measures in order to avoid complications during the fixed orthodontic appliances therapy.

Background: This research aimed to evaluate the salivary concentrations of fluoride (F -), calcium (Ca 2+ ), and phosphate (Pi) after brackets bonding, and to identify the role of [F -], [Ca 2+ ], and [Pi] on the development of active caries lesion (ACL) in individuals under fixed orthodontic treatment. Material and Methods: A longitudinal investigation with twenty-two individuals from 11 to 22 years of age was performed in four phases (baseline and after 1, 3, and 6 months). Analyses were carried out considering the salivary concentration of [F -], [Ca 2+ ], and [Pi], as well as the caries index. Data were analyzed using the Friedman test, followed by the Wilcoxon test and the multivariate Cox model (p≤0.05). Results: 1 and 3 months after appliance bonding, the [Ca 2+ ] was statistically lower than after 6 months (p<0.0083). On the other hand, salivary [F -] and [Pi] did not show any significant difference during the follow-up. The Cox model demonstrated that the increase of 1 µg/mL in Ca 2+ decreased the risk of ACL development by 27%. In conclusion, the levels of Ca 2+ changed during orthodontic treatment. Conclusions: A high Ca 2+ level in the saliva is a protective factor for ACL development over time.

Saliva is a biological fluid of complex composition that facilitates the interactions between host cells, oral microorganisms and immune modulators with the main purpose of maintaining oral health status. Within the salivary components, proteins are fundamental, because they are responsible for the main functions of saliva. Proteomic studies contributed to identify and characterize between 2,000 and 2,600 different proteins and peptides in their composition. Among them, a high proportion is glycoproteins, such as mucins, proteins rich in proline and immunoglobulins, agglutinins, lactoferrin, cystatins and lysozyme. In addition, saliva possesses peptides of potent antimicrobial activity and protection to mineral tissues, such as cathelicidin LL-37, α and β defensins, histatins and statins. In particular, in recent years the selection of biomarkers of oral diseases such as dental caries, periodontal diseases and oral cancer has been focused, considering the simple, economical and non-invasive obtaining of this fluid, which has driven the development of technologies for the detection of biomarkers with high sensitivity and specificity for a wide variety of oral diseases. The study of the components of saliva and its potential biomarkers makes this fluid a powerful tool for diagnosis and detection of diseases, as well as to evaluate the evolution of therapeutic treatments, because it reflects the physiological and pathological state of the body.KEY WORDS: salivary proteins and peptides, biomarkers, dental caries, periodontal diseases, oral cancer ResumenLa saliva es un líquido biológico de composición compleja que facilita las interacciones entre las células del huésped, los microorganismos bucales y los moduladores inmunológicos con el principal propósito de mantener el estado de salud bucal. Entre ellas, una alta proporción son glucoproteínas, como las mucinas, proteínas ricas en prolina e inmunoglobulinas, aglutininas, lactoferrina, cistatinas y lisozima.
