Infrared Matrix-Assisted Laser Desorption Electrospray Ionization Mass Spectrometry Imaging of Human Hair to Characterize Longitudinal Profiles of the Antiretroviral Maraviroc for Adherence Monitoring

Abstract: Here, we assess infrared matrix assisted laser desorption electrospray ionization (IR-MALDESI) mass spectrometry imaging (MSI) analysis of hair as a clinical tool for monitoring patient adherence to the antiretroviral maraviroc (MVC). A custom MATLAB-based algorithm has been developed to streamline data analysis and generate longitudinal profiles of drug incorporation along the length of hair strands. Hair strands from volunteers enrolled in a directly observed therapy study were analyzed by IR-MALDESI MSI and… Show more

“…Currently, the following methods are used in China and other countries to determine adherence in AIDS patients: (1) self-reported evaluation questionnaires [ 9 , 11 , 13 , 14 , 16 – 20 ], (2) tablet, prescription and pharmacy records for drug count [ 21 , 22 ], (3) plasma drug concentration monitoring [ 10 , 22 , 23 ] and (4) a mixture of self-reported questionnaires and plasma drug concentration monitoring. However, method (1) tends to be limited by patient-related subjective factors, and its reliability and validity tend to be questionable.…”

“…Quantitative MSI analysis of HIV antiretrovirals in human hair strands has also been reported, with LODs less than or equal to 1.6 ng/mg hair and linear response relative to LC‐MS/MS ranging three orders of magnitude (Rosen et al, 2016). The feasibility of IR‐MALDESI as a clinical tool was further demonstrated in monitoring patient adherence to the antiretroviral maraviroc (MVC) (Gilliland et al, 2019). Using cholesterol to identify the location of the hair strands, MVC was detected along the length of hair strands 28 days after single or daily doses.…”
