Inhaled lipid cisplatin (ILC) in the treatment of patients with relapsed/progressive osteosarcoma metastatic to the lung

Abstract: ILC is well tolerated in heavily treated osteosarcoma patients and did not appear to have the typical toxicities associated with intravenous cisplatin. Three of eight patients with less bulky disease had sustained benefit. Further study of ILC is warranted.

“…Inhalation therapy has benefits for treatment of lung diseases. Indeed, direct delivery of the drug avoids first-pass liver metabolism, enabling effective administration and larger distribution of the active molecule into the lung, while limiting anatomical filters [142]. Intranasal deposition of complexes also allows a quick assessment of the in vivo efficiency of a nonviral formulation, in a noninvasive manner, as nasal epithelium responds similarly to lung epithelium.…”

“…To be effective, an inhaled agent should reach the lung in sufficient concentration, with the induction of minimal pulmonary toxicity and limited systemic absorption inducing systemic side effects [142]. Nevertheless, this pathway shows some disadvantages.…”

“…CP presents a less toxic profile, in comparison to cisplatin [47]. Cisplatin has been tested in animals and humans as aerosol [37,38,54,55]. The physicochemical properties of aerolised drugs (for example: molecular weight, shape, charge, PH, mass median diameter of aerosol particles and aqueous solubility) determine the rate of diffusion through the respiratory tract.…”

“…grade 3 vomiting, severe dyspnea with chest pain, and worsening of malignant pleural effusion. Out of 19 patients, only patients with lesions smaller than 2 cm or that underwent resection of the lung disease experienced a sustained benefit from the inhaled liposomal cisplatin formulation: two patients achieved a complete response after metastasectomy, one patient achieved a complete response after the inhalation treatment, and one had a sustained partial response, other patients discontinued the drug due to progression of disease (Chou et al, 2013).…”
