Inhaler devices in asthma and COPD patients – A prospective cross-sectional study on inhaler preferences and error rates

Abstract: Background: Inhalation therapy is the backbone of asthma and COPD control. However, inhaler adherence and device mishandling continue to be a problem in real life. Some studies have shown that using a patient-preferred inhaler may reduce device handling errors and improve adherence to prescribed chronic inhaler drug therapy. The aim of this study was to compare the preferences for commonly used inhaler devices in Germany in patients with chronic obstructive respiratory disease. We also pursued the question whi… Show more

“…This inhaler attribute in combination with an easy cleaning procedure is also an important factor to ensure patients' handling satisfaction 25 . Today it is well accepted, that therapeutic success of inhalation therapy depends more than expected on a patient's perception, preference, and satisfaction with the inhaler 26,27 because higher satisfaction with the device seems to contribute markedly to an increased treatment adherence in COPD patients 3,[28][29][30] .…”

“…The lowest number of attempts to achieve error-free use was reported for the Turbuhaler ® and Nexthaler ® devices. Overall, Spiromax ® and Nexthaler ® achieved the best overall ratings and where the devices most preferred by patients [41], 1.02 should be replaced by 1.10. Table 1 lists clinical trials conducted with the Spiromax ® , Turbuhaler ® , and Easyhaler ® devices.…”

“…In a recent study investigating patients' preferences for inhaler device types, 10 different placebo inhaler devices were presented to patients. Despite a high proportion of patients being MDI users, only 14.3% selected MDI as their preferred device, with multidose DPIs proving most popular 51 . This suggests that prescribers should generally offer DPIs or SMIs as the first choice.…”

“…Most patients are able to use a DPI, even during exacerbations 52–54 . Given a choice of modern devices, most patients will select a multidose DPI as their first‐choice inhaler 51 . Some patients (in particular very young children with asthma and elderly patients with severe COPD) may prefer or require an MDI with a spacer.…”
