Initiating Extradural Analgesia during Labour: Comparison of Three Different Bupivacaine Concentrations Used as the Loading Dose

Abstract: Objective: Potential effects of extradural analgesia on the progress of labour and obstetric outcome are still a matter of concern and the focus of ongoing debates. Despite this, little attention is paid to the initiation of extradural labour analgesia. The objective of this prospective, randomized, double-blind trial was to identify the optimal of three concentrations of bupivacaine used as a loading bolus for initiating extradural analgesia during labour. Methods: Sixty-seven full-term parturients requesting… Show more

“…Studies by Justin et al .,[ 9 ] stated that local anaesthetics and opioid combination were shown to be more effective in labour analgesia as effects started rapidly and lasted longer when compared with local anaesthetics given alone. [ 10 11 12 ] Similarly when local anaesthetics are used in low concentration, sufficient analgesia cant be obtained especially in second stage of labour. [ 13 ]…”

“…We hypothesised that the small, frequent top-up technique would be an acceptable alternative to CEI in respect of the quality of analgesia, overall maternal satisfaction and safety and would be preferable to the traditional large, intermittent top-ups. We have used a mixture of 0.125% of bupivacaine and 2 µg/ml of fentanyl for all three epidural analgesia techniques, as 0.125% has been found to be the ideal concentration 6 . Fentanyl was added because a combination of an opioid with local anaesthetic reduces the local anaesthetic requirement, hastens the onset of action and provides intense analgesia without affecting bearing-down ability.…”
