Insertion of intrauterine devices after cesarean section: a systematic review update

Abstract: BackgroundWomen who undergo a cesarean section (CS) are in a unique position to receive the intrauterine contraceptive device (IUD). They may also want to use the IUD as a long-acting reversible contraceptive method provided the IUD is safe and effective in the presence of a CS scar.Search strategyWe researched and reviewed the MEDLINE, POPLINE, Google Scholar, and ClinicalTrials.gov databases from January 1968 to June 2015.Selection criteriaEligible studies reported event rates or practical problems relating … Show more

“…Involution of the uterus makes IUCD strings to descend and are visible vaginally. 8 The IUCD string is used to locate the device in utero and to remove the device. Lost strings occurs due to expulsion, curling and in-drawing into the uterine cavity, embedding, breaking of the strings, uterine perforation and translocation into abdominal cavity.…”

“…2 IUCDs have a rapid onset of action after insertion with a rapid return to fertility after discontinuation. 8 Recognizing the potential impact of improved family planning programming on maternal and child health, Government of India is committed to expanding access to family planning towards achieving Millennium Development Goals 4 and 5. 9 Hence postpartum intrauterine contraceptive device (PPIUCD) use is being encouraged.…”

“…In Nepal, the postpartum intrauterine device (PPIUD) is the sole long acting reversible method available in the immediate postpartum period [6]. PPIUD can be used immediately after childbirth, with minimal side effects for up to 10 years [7,8]. However, its uptake in Nepal is lower than in many other countries, which has largely been attributed to societal barriers and a lack of knowledge and skills among health providers [1,9].…”

“…A previous study has identi ed female community health volunteers (FCHVs) in Nepal to play a key role in in uencing a woman's decision to choose a PPFP method. It is therefore important that we understand the long-term impact of including this cadre of health worker in PPFP interventions [8].…”

“…IUD insertion at the time of CS creates an opportunity to increase access to long-acting reversible contraceptive methods. Conversely, a previous CS scar may deter access to interval insertion of an IUD if a previous CS may result in difficulty with insertion and/or future IUD problems [1] .…”
