Intent‐to‐treat survival in liver transplantation has not improved in 3 decades due to donor shortage relative to waitlist growth

Abstract: Background: Liver Transplantation has advanced over the past 3 decades, with 1-year survival rates improving 25%. Survival rates for those transplanted has increased to remarkable levels, but survival from the time of listing may not be as revolutionary. Methods: Kaplan-Meier with log-rank test as well as Cox regression analysis was used to retrospectively analyze 211 610 adults listed for LT and 116 299 adult transplant recipients from 1987 to 2016. Our primary endpoints were survival from time of listing to w… Show more

“…Liver transplantation is the ultimate life-saving treatment for end‐stage liver diseases to date, which has been hampered by a critical shortage of organs 1 . As an alternative to donor organs, many bioinspired approaches have been widely explored for liver tissue engineering, such as cell encapsulation, 3D printing and hepatic organoids, in combination with natural or synthetic materials (e.g., hydrogel, alginate, gelatin methacryloyl (GelMA), and polycaprolactone) 2 , 3 .…”

“…The success of liver transplantation (LT) in the treatment of end-stage liver failure has resulted in an important imbalance between the numbers of patients on the waiting list and donor organ availability [ 1 ]. To overcome this imbalance, transplant clinicians are accepting more high-risk liver grafts from extended criteria donors, presenting characteristics such as advanced age or hepatic steatosis, as well as donation after circulatory death (DCD) donors, which are exposed to a period of warm ischemia (WI) prior to procurement, aggravating subsequent ischemia-reperfusion injury (IRI) [ 2 , 3 ].…”

“…Since the first case of liver transplantation (LT) in 1963, there has been huge progress that has dramatically increased in the 1-year LT survival rates from 86% in the 1980s to 93% in 2016 (Dunson et al, 2021). Liver transplantation remains a therapeutic option for many patients with liver cirrhosis, hepatocellular carcinoma and treatment-refractory end-stage liver disease (Samuel & Coilly, 2018).…”
