International variation in the management of infants hospitalized with respiratory syncytial virus

Abstract: Management of RSV patients varies markedly by country and hospital. Multicenter RSV trials that measure length of stay should standardize criteria for "readiness for discharge". It may be appropriate to limit international trials to countries with similar median stays for RSV. Variability within multicenter trials could be further controlled by standardizing the use of other therapies and the diagnosis of complications.

“…Despite this recommendation, previous reports indicate that up to 60% of infants with severe bronchiolitis receive corticosteroid therapy. 24,25 Our finding that Hispanic children with severe bronchiolitis were most likely to be discharged on inhaled corticosteroids is potentially concerning, as it exposes a subset of children to treatment that is not recommended. On the other hand, given the increased risk of future asthma in Hispanic communities, a higher use of inhaled corticosteroids might be seen as appropriate.…”

“…Examples of such supportive treatments include bronchodilator use, supplemental oxygen, intravenous hydration, parenteral antibiotic, systemic corticosteroid, intensive care, mechanical ventilation, and respiratory physiotherapy [11]. Despite many years of research, physicians are still unclear about how to optimally manage bronchiolitis patients in various care settings (ED, hospital, and outpatient), including which treatment model is the optimal one for a specific bronchiolitis patient [2,4,10,241].…”
