Sonography has proved to be an important diagnostic and operative instrument in an in-vitro fertilization and embryo transfer (IVF/ET) program. In some IVF/ET programs the technique has even replaced laparoscopy for oocyte retrieval. To date, transvesical follicle aspiration has been the main technique for ultrasound-guided oocyte pick-up. However, this technique has some disadvantages. Therefore, we have developed a transvaginal ultrasound-guided puncturing technique. A new high-frequency vaginal sector transducer was utilized (Brüel & Kjaer). This new transducer, equipped with a needle guide, changed the oocyte retrieval into a simple, safe, and rapid procedure that can be performed under local anesthesia without any discomfort for the patient.

Complications related to approximately 8000 ultrasound-guided punctures performed in our institution during the last 19 years are reported. This study includes 3500 fine needle biopsies, 700 large bore needle biopsies, 2800 punctures of fluid collections using either a 1.2-mm spinal needle or a 7.5 F pigtail catheter, and 1000 percutaneous nephrostomies. The majority were abdominal punctures. The complication rate was 0.187% (0.100% to 0.313%) and the mortality rate 0.038% (0.013% to 0.113%). Using the fine needle data exclusively, these rates were 0.200% (0.086% to 0.400%) and 0.028% (0.000% to 0.171%), respectively. The 95% confidence limits are in brackets.

First trimester fetal diagnosis was established in 100 pregnancies at risk by transabdominal chorionic villus sampling (TA-CVS). Forty-eight per cent of the women were 35 years or more at the time of sampling. Using the double needle technique, both the aspiration and the diagnostic success rate were 100 per cent. The mean amount of villi aspirated was 28.2 mg (10-50 mg). The mean needle time was 3 min. Vaginal spotting appeared in 2 per cent of the women. Four women had therapeutic abortion due to abnormal findings and one for social reasons. Three fetuses with normal karyotypes were lost. Excluding the therapeutic abortions, the fetal loss rate was 3.2 per cent. The fetal loss rate in the amniocentesis control group (n = 200) was 3.6 per cent. The cytogenetic diagnosis was carried out by the direct preparation technique as well as by chorion villus cultivation. All karyotypes were confirmed by lymphocyte cultures from umbilical cord blood or heel blood from the newborn or from aborted fetal tissue. Transabdominal CVS is deemed a safe and easy tool for achieving chorionic villi in the first trimester.
