Intestinal ischemia: current treatment concepts

Abstract: Patients with acute mesenteric ischemia are still at highest risk for a fatal course of disease. New diagnostic and therapeutic developments have not been tested in larger studies yet, neither has any of these methods led to an increased survival in studies published so far. Taken together, mesenteric ischemia requires high awareness, earliest possible diagnosis, and treatment by an experienced interdisciplinary team of gastroenterologists, radiologists, and surgeons.

“…[45][46][47][48][49] The clinical picture in the early stages is unspecific and similar to other abdominal syndromes (peritonitis, occlusion). Pain can be acute, sudden (arterial embolism or thrombosis) or slowly progressing (ischemia by non occlusal causes or venous mesenteric thrombosis), associated with nausea, vomiting and change in bowel habits.…”

“…In this case, there are abdominal findings that are not in proportion with physical examination and they do not adequately aid in the early diagnosis of AMI (5,6). There are several diagnostic methods; however, the essential factors that reduce mortality are early diagnosis and appropriate treatment (7). Even though computerized tomography (CT) has a high sensitivity for diagnosis, some technical problems limit the use of CT-angiography such as renal failure and serious allergic reaction (8).…”

“…Hypoxia is a common pathological phenomenon, accompanying with many diseases, such as neurologic complications, 1 ischemic heart disease, 2 acute renal failure, 3 intestinal ischemia, 4 pulmonary disease, 5 carbon monoxide poisoning, 6 traumatic brain injury, 7 perinatal asphyxia, 8 sudden cardiac death, 9 etc. Severe hypoxia often leads to death.…”
