Intradialytic hypotension (IDH) is an adverse event occurred during hemodialysis (HD) sessions with high morbidity and mortality. The key to preventing IDH is predicting its pre-dialysis and administering a proper ultrafiltration prescription. For this purpose, this paper builds a prediction model (bCOWOA-KELM) to predict IDH using indices of blood routine tests. In the study, the orthogonal learning mechanism is applied to the first half of the WOA to improve the search speed and accuracy. The covariance matrix is applied to the second half of the WOA to enhance the ability to get out of local optimum and convergence accuracy. Combining the above two improvement methods, this paper proposes a novel improvement variant (COWOA) for the first time. More, the core of bCOWOA-KELM is that the binary COWOA is utilized to improve the performance of the KELM. In order to verify the comprehensive performance of the study, the paper sets four types of comparison experiments for COWOA based on 30 benchmark functions and a series of prediction experiments for bCOWOA-KELM based on six public datasets and the HD dataset. Finally, the results of the experiments are analyzed separately in this paper. The results of the comparison experiments prove fully that the COWOA is superior to other famous methods. More importantly, the bCOWOA performs better than its peers in feature selection and its accuracy is 92.41%. In addition, bCOWOA improves the accuracy by 0.32% over the second-ranked bSCA and by 3.63% over the worst-ranked bGWO. Therefore, the proposed model can be used for IDH prediction with future applications.

Background: The prevalence of chronic kidney disease (CKD) is increasing worldwide. The progression of CKD aetiology increases the probability of death, which lowers quality of life (QOL). Therefore, we studied complications encountered in CKD patient undergoing hemodialysis at tertiary health center. Methods: This is a cross sectional, observational study conducted among 100 patients for 24 months duration. All CKD patients who were undergoing maintenance hemodialysis at tertiary health care center in Vidharba, India. Patients with CKD (stages 3-5) who required hemodialysis, regardless of gender, were included, as were all patients older than 12 years old. Results: We studied 100 CKD patients undergoing hemodialysis in which 33 (33%) were females and 67 (67%) were males. Hypotension was the most common intradialytic complication (34.2%), followed by muscle cramps (19.5%) and rigors (17.5%). Other complications such as hypoglycemia, nausea, vomiting, chest pain, itching, arrhythmias, and pulmonary edema were reported at lower frequencies. The study found no significant association between age, gender and intradialytic complications. (P value= 0.072). Conclusion:Based on the outcomes of the study, we can conclude that hypotension is the most frequent intradialytic complications followed by, hypoglycemia, nausea/vomiting, muscle cramp, rigors, chest pain, itching, arrhythmias and pulmonary edema. Majority of patients were found to be comorbid having hypertension, diabetes which again are the factors responsible for worsening of disease.

Background: Intradialytic hypotension (IDH) is a common and distressful complication of hemodialysis. It can cause uncomfortable symptoms for patients, lower dialysis efficiency, and result in vascular access thrombosis. Therefore, preventing this complication is essential. Aim: To evaluate the effects of cooling the dialysate and patient education on decreasing intradialytic hypotension. Design A Quasi-experimental study design was used. Methods A purposive 70 patients undergoing hemodialysis in a hospital in Egypt participated in the study. Tool I (Structured Interviewing Questionnaires Sheet) and Tool II (Intradialytic hypotension Assessment Sheet) were used to collect data. Interventions include educate patients some instructions help in decrease IDH. In addition, lower dialysate temperature. Results A statistically significant difference in the occurrence of intradialytic hypotension was observed between pre and post intervention during every hour of hemodialysis (p < 0.01). Also, more than half of the patients reported symptoms of intradialytic hypotension before intervention, including fatigue, sweating, cramps, and dizziness with a statistically significant difference from those after intervention (p = 0.001). Conclusion: As a result of the intervention done, the symptoms observed during dialysis and the prevalence of IDH decreased. Recommendations: Cooling the dialysate and educating patients instructions regarding diet and dealing with drugs can be routinely applied to prevent IDH in hemodialysis units.
