Intraductal Papillary Neoplasms of the Pancreas: Diagnosis by Endoscopic Pancreatic Biopsy

Abstract: Intraductal papillary neoplasms of the pancreas (IPN) are rare tumors, distinct from other pancreatic duct cell carcinomas both in terms of papillary polyp formation in the main pancreatic duct and their favorable prognosis. In the present study, endoscopic pancreatic biopsy (EPB) under fluoroscopic control was employed in three cases of IPN to obtain a tissue diagnosis prior to surgery. There were no complications and all biopsy specimens were adequate for histopathological evaluation. In our experience, EPB … Show more

“…5 Determination of the malignancy grade is difficult even with biopsy specimens obtained directly from the tumor, because of histological heterogeneity. 6 Several investigators have insisted that IPMT can be categorized into two subtypes, depending upon which part of the pancreatic duct is involved. 7,8 That is, in comparison to tumor involving the branches, tumor involving the main duct is usually associated with greater progression of histological features.…”

“…Other investigators have attempted to preoperatively differentiate benign from borderline/malignant MPT 22. Unfortunately, malignancy was often not clearly defined preoperatively and the diagnosis may have been based only on the degree of cytological atypia 12,23. We decided that surgery was indicated for MPTs with the following features: (1) main pancreatic duct >7 mm in diameter, (2) a cystic lesion >25 mm in diameter, and (3) a nodule in a cystic lesion >6 mm in diameter 23.…”

“…Mucin‐producing tumors (MPTs) of the pancreas have unique clinical features distinct from those of “common pancreatic ductal cell carcinoma”, and patients have a relatively good prognosis after surgical removal . It is possible that MPTs are being increasingly recognized because of the more frequent use of computed tomography (CT), ultrasonography (US), and endoscopic retrograde cholangiopancreatography (ERCP) 12,13. This disease is known by a variety of names which have been summarized elsewhere 14,15.…”
