Intraoperative functional MRI as a new approach to monitor deep brain stimulation in Parkinson's disease

Abstract: This article deals with technical aspects of intraoperative functional magnetic resonance imaging (fMRI) for monitoring the effect of deep brain stimulation (DBS) in a patient with Parkinson's disease. Under motor activation, therapeutic high-frequency stimulation of the subthalamic nucleus was accompanied by an activation decrease in the contralateral primary sensorimotor cortex and the ipsilateral cerebellum. Furthermore, an activation increase in the contralateral basal ganglia and insula region were detect… Show more

“…Thus far, DBS-fMRI studies have been conducted in rats (Lai et al, 2015; Lai et al, 2014; Shih et al, 2014; Van Den Berge et al, 2015; Yang et al, 2013), pigs (Knight et al, 2013; Min et al, 2012; Paek et al, 2015) and humans (Hesselmann et al, 2004; Jech et al, 2001; Kahan et al, 2014; Phillips et al, 2006; Rezai et al, 1999) using gradient-echo echo planar imaging (GE-EPI). For optimal contrast based on the blood oxygenation level dependent (BOLD) effect (Kim and Ogawa, 2012; Ogawa et al, 1990), the echo time (TE) of the EPI sequence is usually set equal to the transverse relaxation time.…”

“…Nuttin et al [8] proposed an excitation of frontopontine pathways under DBS, which was also seen in our case (BA 9 and 10). Although the striatal activation may be related to stimulation, its needs to be distinguished from artifactual activation near the electrode with alternate stimulation [15,20]. Compared to our study, Nuttin et al [8] used a higher stimulation current (10.5 V) and the internal capsule as target.…”

“…Thus, intraoperative fMRI may provide a new technique to assess changes in cortical activity distant from the stimulating electrode comparable to the findings of patients with Parkinson's disease that are treated by DBS [15]. In the case of DBS of the subthalamic nucleus, a decrease in activation in the primary motor cortex could be seen, separate from the stimulated target.…”
