Intraoperative transesophageal echocardiography assessment of right atrial myxoma resulting in a change of the surgical plan

Abstract: Transesophageal echocardiography (TEE) is an important diagnostic tool. It provides structural and functional assessment of cardiac structures which can improve the overall outcome of the patient. We present a case with right atrial myxoma in which TEE helped to find the attachment of the mass so that overall surgical plan was changed.

“…Finally, transesophageal echocardiogram (TEE) can provide excellent views as in our case, assess the mobility of the mass, and can also determine the location of the tumor and its relation to other major landmarks such as the valve apparatus. [9] The role of TEE is pivotal as it provides vivid images of the mass and facilitates a surgeon–operator interaction for optimal decision-making and management. TEE also confers the benefit of detecting small masses or unclear cardiac structures, especially when a transthoracic echocardiogram is suboptimal.…”

“…Echocardiographic detection of a pedunculated, smooth mobile mass sitting in the LA is almost pathognomonic of LA myxoma [19]. Transesophageal echocardiography (TEE) can detect masses as small as 1-3 mm and provide more details about the exact location, dimensions, and appearance of the tumor, but it is not used routinely for diagnosis, it is reserved for cases of unsure diagnosis or intra-operative assessment [20]. In our series, the tumors were diagnosed in all cases by TTE.…”

“…Most reports describe intracardiac masses such as thrombi, lipomas, and myxomas as well as masses of unknown etiology [ 1 ]. Fewer reports exist on the echocardiographic assessment and surgical guidance on the resection of intracardiac masses [ 2 , 3 ]. Only a handful of reports exist describing cardiac compression from external masses [ 4 6 ].…”
