Intrauterine development of posthaemorrhagic hydrocephalus

Abstract: Differential diagnosis of intrauterine hydrocephalus includes various types of cerebral malformations. In about 30% of cases the pathogenesis remains unknown. During 1983-1985 we diagnosed intrauterine posthaemorrhagic hydrocephalus (IPHH) in four newborns. In three of the reported cases intraventricular haemorrhage (IVH) could already be proven in the fetus. Due to these observations, IPHH has to be considered with the differential diagnosis of congenital hydrocephalus. Haemorrhagic diathesis and multiple pre… Show more

“…This association is not uncommon [4][5][6][7][8][9][10] , and it is usually explained on the basis of hypoxia leading to both IUGR and intracerebral circulatory changes with secondary hemorrhage 4 . However, it might also be hypothesized that 33 .…”

“…As many of these cases were associated with placental abruption 2,3 or intrauterine growth restriction (IUGR) [4][5][6][7][8][9][10] , the same hypoxic / asphyctic pathogenesis that is often relevant in newborns was hypothesized to be so also in fetuses. In other cases, fetal intracranial hemorrhage has been linked to fetal thrombocytopenia 7,11 or coagulation disorders 12 -14 . Hemorrhage in a subdural location has been mainly related to maternal trauma, often also causing uteroplacental or other fetal injuries 15 .…”

Although hemorrhages evident at different ages share many risk factors, preliminary analyses have led us to view early-onset (before 12th postnatal hour) germinal matrix hemorrhage (EGMH) as etiologically distinct from later-onset germinal matrix hemorrhage .…”

“…The causes of fetal hydrocephalus are varied and have prognostic significance (5,32). In utero hydrocephalus as a consequence of identifiable hemorrhage has been well documented, the majority in the third trimester by readily identifiable hemorrhagic lesions (14,18,25,31,37). Cryptic intracerebral hemorrhage has been postulated as a cause of fetal hydrocephalus (8,10,12); however, by definition, it cannot be diagnosed by current ultrasound examination techniques.…”
