Intravenous Branched Chain Amino Acid Trial in Marrow Transplant Recipients

Abstract: Branched chain amino acids (BCAA) improve nitrogen balance and end-organ function in surgical patients, but are untested in marrow transplant recipients. We compared nitrogen balance, urinary 3-methylhistidine-to-creatinine ratio, upper arm anthropometry, serum prealbumin, and day to peripheral engraftment in a randomized, double-blinded trial between 45% (high-leucine) and 23% BCAA intravenous solutions in 40 adult leukemia patients for 1 month following allogeneic marrow transplantation. Nutritional support,… Show more

“…So termination of PN may result in loss of weight and muscle mass. 6,21 Hematologic outcomes Our study showed that time to platelet engraftment was shorter in group A than group B. It was revealed in murine studies that engraftment was better with feeding than with fasting.…”

“…Because such infections are usually treatable bacterial infections, in contrast to fungal and viral infections, which usually occur after engraftment and discontinuation of PN. 6,21 In addition, chemotherapy, radiotherapy and medicine prescribed to hematopoietic SCT patients may suppress the immune system, which renders hematopoietic SCT patients prone to infection. On the other hand, because of buccal mucositis, esophagitis and enteritis, hematopoietic SCT patients are unable to tolerate oral intake.…”

“…Based on this premise, numerous projects studying the effect of BCAA in the critically ill have been published [1][2][3][4][5][6][7][11][12][13][14][15][16][17]. A majority of these studies have used nitrogen balance as their gold standard in terms of evaluating the benefit of BCAA.…”

“…A majority of these studies have used nitrogen balance as their gold standard in terms of evaluating the benefit of BCAA. With this as a criterion, several studies have shown a positive effect for BCAA in critically ill patients [1][2][3][4][5][6][7], while others have shown no beneficial effects [11][12][13][14][15][16][17]. It is worth noting that in none of these studies were detrimental effects of BCAA documented.…”
