Intravenous dipyrone for the acute treatment of episodic tension-type headache: A randomized, placebo-controlled, double-blind study

Abstract: Acute headaches are responsible for a significant percentage of the case load at primary care units and emergency rooms in Brazil. Dipyrone (metamizol) is easily available in these settings, being the most frequently used drug. We conducted a randomized, placebocontrolled, double-blind study to assess the effect of dipyrone in the acute treatment of episodic tension-type headache. Sixty patients were randomized to receive placebo (intravenous injection of 10 ml saline) or 1 g dipyrone in 10 ml saline. We used … Show more

“…Very good effects have been attained in treatment of acute pain using metamizole in combination with NSAIDs, including ketoprofen (Oberhofer et al 2005) or with opioid analgesics (López-Muñ oz et al 2008, Baumgartner et al 2009). The efficacy of metamizole in post-operative pain therapy has been confirmed in studies conducted on humans (Edwards et al 2001, Chaparro et al 2012) and on animals (Imagawa et al 2011). Bigal et al (2002 determined that metamizole administered intravenously was effective for the acute treatment of episodic tension-type headache.…”

“…Compared to other non-opioid analgesics, metamizole seems to be a relatively safe drug (Bigal et al 2002, Imagawa et al 2011. The most common adverse effects are: gastrointestinal disorders, such as nausea, vomiting, abdominal pain and diarrhea (Edwards et al 2001).…”

“…Dipyrone, the main constituent of the combination product, has been evaluated extensively since its commercial introduction in 1922; oral or intravenous dipyrone has demonstrated efficacy in relieving episodic TTH or migraine headache pain [6,[32][33][34]. Indeed, dipyrone has also been the subject of controversy and discussion over the years, owing to reports in the 1950s and 1960s of a high incidence of agranulocytosis; in more recent years, the abnormally high incidences that were reported in the earlier studies have been apportioned to imprecise studies of adverse effects, and projected cases of agranulo cytosis with dipyrone are similar, or even lower than those with some other analgesic drugs [35].…”
