Intussusception: indications for ultrasonography and an explanation of the doughnut and pseudokidney signs

Abstract: Ultrasonography has been shown to be valuable in the detection of intussusception, but a question arises, as to just when this study should be performed. Should it be a general screening procedure or should it be utilized for specific cases only? Upon reviewing the literature, and the findings in 14 of our patients, we feel that it should be utilized as a general screening procedure. In addition, we offer a different explanation for the typical doughnut and pseudokidney signs seen with intussusception.

“…At US, an intussusception may have a -doughnut‖ or -pseudokidney‖ appearance on images obtained transversely or longitudinally to the long axis (14). The hypoechoic outer area represents the edematous bowel wall of the intussuscipiens and the hyperechoic central area represents mesenteric fat that is dragged into the intussusception between the entering and returning limbs of the intussusceptum.…”

“…We feel, like others, that ultrasonography is a reliable, quick and noninvasive method to confirm or exclude intussusception, and should be the first line of investigation, especially in those with atypical clinical presentation. 14,15 Intussusception in the majority of children between the fifth and ninth month of life is said to be idiopathic, i.e., unrelated to any pre-existing anatomical abnormality. 1 This type of intussusception accounts for 90%-95% of the total number of childhood intussusceptions, and although some of these patients have an anatomical lead point, the majority have idiopathic intussusception.…”
