Invasive Squamous Cell Carcinoma of the Conjunctiva First Misdiagnosed as a Pterygium: A Clinicopathologic Case Report

Abstract: All pterygia should be evaluated meticulously with regard to possible underlying causes, such as carcinoma in situ or squamous cell carcinoma, and all excised lesions should also be evaluated pathologically to prevent such serious adverse outcomes that are mentioned in this case report.

“…This finding must be placed in the context of the potential catastrophic repercussions of undiagnosed OSSN. [24][25][26] The rate of concomitant OSSN in pterygia specimens varies worldwide from 0% to 9.8% as summarized in Table 1. In a retrospective analysis of 2005 pterygia pathology records in Miami, FL, 1.7% were found to have OSSN of which 41% were clinically ''suspected'' preoperatively and 59% were unexpectedly found on histopathology.…”

“…OSSN can occur with corneal ulcer [16], chronic blepharoconjuctivitis [17], pterygium [6,19,20], necrotizing scleritis [21,22], sclerokeratitis[18,29], orbital cellulitis [23], superior limbic keratoconjuctivitis [24], ocular prosthesis [25,26], fibroxanthoma[27], and MMP [28]. In the published literature, several cases of the OSSN were not recognized clinically, but were found when biopsies were done for other presumed diagnoses.…”
