Investigations of thyroid hormones and antibodies based on a community health survey: the Busselton thyroid study

Abstract: This study establishes the prevalence of antibodies to thyroperoxidase and thyroglobulin in a community-based sample and reference intervals for free T4 and TSH. When the NACB decision limits are applied to older men or women, there is a markedly increased number with 'elevated' autoantibody levels compared to sex- and age-specific reference intervals.

“…In our study, exclusion of at-risk individuals increased the lower bounds of the 95% TSH limit, but had no effect on the mean and upper range fT 4 concentrations in groups according to gender and age. In addition, the prevalence of mild hypothyroidism (TSH )4.5 mU/L) was approximately three-fold more common in females than males, consistent with earlier studies (11,24). Most studies established one reference interval for a population, as recommended by the NACB (8).…”

“…Certainly, TPOAb is more relevant than TgAb among those with elevated TSH in reports from Europe and the United States [2,16,17,18,19]. The prevalence of TgAb is higher than that of TPOAb in patients with Hashimoto's disease in reports from Japan [4,5], but not in the report from Europe [7].…”

“…In recent studies, similar prevalence of positivity, 12.4% to 14% (35,43,106) Although the presence of TPOAb usually precedes the development of thyroid dysfunction, recent studies suggest that a hypoechoic ultrasound pattern may precede a biochemical TPOAb abnormality (108,109) (A,B) The paradoxical absence of TPOAb in some patients with unequivocal TSH abnormalities likely reflects the suboptimal clinical and methodological sensitivity and/or specificity of current TPOAb tests (25,109) (B,B).…”
