Is early BCG vaccination associated with less atopic disease? An epidemiological study in German preschool children with different ethnic backgrounds

Abstract: We investigated the association of bacille Calmette-Guérin (BCG) vaccination and atopic manifestations among children. Because many children in the study area were from minority ethnic groups, the effect of ethnicity on disease prevalence was also analyzed. A mandatory health survey included all preschool children from Berlin in 1994. Trained medical personnel asked parents whether their child had ever had a diagnosis of atopic dermatitis (AD), bronchial asthma (BA), and hay fever (HF) or symptoms suggestive o… Show more

“…In developed countries, many studies found an absence of any relationship between BCG vaccination and atopic diseases in children (1011-1017) and young adults (1008). In other developing countries, there was a weak protective effect of BCG vaccination against asthma and hay fever (1018)(1019)(1020). In developing countries, an early BCG vaccination was associated with a weak prevention of atopic diseases (1009).…”

“…In contrast to what was expected, most of the epidemiological studies, performed mainly in central and northern European countries as well as in Australia, show no effect of BCG immunization on the prevalence of asthma or atopy either when the vaccine is taken early in life [10,11,12,13,14] or when it is administered during adolescence [15, 16]. However, there seems to be a different result when the studied population originates from a different ethnic background: some investigations have shown that BCG immunization could in fact offer some protection in some immigrant populations [14, 17] in western countries or in less developed countries such as Guinea-Bissau [18]. …”

“…The only study showing a somewhat protective effect of BCG immunization on asthma in a western country is the large German study by Gruber et al [17] which found a weak but statistically significant protective effect of this immunization on asthma but not on hay fever or atopic dermatitis. This study surveyed children from East and West Germany.…”

“…(4) Other authors have also failed to find any association between BCG vaccination and a lower prevalence of asthma. (22,23) One study showed that neither BCG vaccination nor PPD positivity has any effect on asthma prevalence. (24) Conversely, one group of authors found that neonatal BCG vaccination was associated with a 27% reduction in the subsequent prevalence of asthma symptoms.…”
