Is it feasible to diagnose germ-cell tumors by fine-needle aspiration biopsy?

Abstract: Fine needle aspiration biopsy is an extremely useful procedure for the diagnosis of germ cell tumors. Most of the subtypes can be easily recognized and differentiated from other subtypes. For difficult cases clinical pathologic correlation may be extremely helpful in establishing a precise diagnosis. Mixed germ cell tumors may cause some difficulty if one of the components is predominant. Immunohistochemistry may also play an important role in arriving at a definitive diagnosis. Diagn. “…The anaplastic-looking cells in UDC may be reminiscent of RS cells, but RS cells always have some cytoplasm, and the brisk apoptosis seen in UDC is not seen in Hodgkin's disease. 8 Although the nuclear features of germ-cell tumors and UDC were found to overlap, the glandular and papillary formations, cytoplasmic vacuoles, and hyaline globules seen in YST 21,22 are not seen in UDC. In a situation where the cytologist is interpreting the smear without clinical details or when metastasis to neck node is a primary mode of presentation, these are helpful features.…”

“…124 Intraoperative imprint cytology may aid in the correct diagnosis of germ cell tumors as they allow for more thorough sampling of the tumor than a single frozen section. 126,127 The nuclear membranes are irregular with coarsely clumped to vesicular chromatin and one or two, large, eosinophilic nucleoli. 68 When positive, APW specimens are typically cellular with large tumor cells that display scant to moderate amounts of indistinct, fragile, vacuolated cytoplasm.…”

“…125 Dysgerminoma Dysgerminomas are the most common malignant germ cell tumor of the ovary, and comprise 1-2% of all malignant ovarian tumors. [126][127][128] While sarcoid-like granulomas and mature lymphocytes may be observed in surgical pathology specimens and in FNA specimens of dysgerminomas, these may not be observed in positive APW. 126,127 The nuclear membranes are irregular with coarsely clumped to vesicular chromatin and one or two, large, eosinophilic nucleoli.…”

“…39,78,81,90 This is because of the marked morphologic overlaps of these tumors. Moreover, it is not uncommon to find a mixture of various germ cell tumor components within one lesion.…”
