Is it mandatory to examine synovial fluids promptly after arthrocentesis?

Abstract: Fifty synovial fluid (SF) samples from patients with various types of arthritis were examined promptly after joint aspiration and after storage at room temperature (22°C) or at refrigerator temperature (4°C) for 1 hour, 2 hours, 3 hours, 6 hours, 1 day, and 3 days, then weekly for 3 weeks and monthly for 2 months. We found that the leukocyte count (white blood cell [WBC] count) decreased within a few hours. In 4 SF samples from patients with mild inflammation (initial range 3,150-6,200 WBC/mm3), the WBC count … Show more

“…Synovial fluids were maintained for at least 48 h without morphological change. Even after one to several weeks of storage at 4°C, the crystal and leukocyte concentrations only tend to be minimally reduced [12,[14][15][16][20][21][22]. Therefore, we think that our storage handling had none or minimal influence on crystal recognition.…”

“…Un ritardo nella lettura del LS può infatti essere causa della dissoluzione dei cristalli di UMS e soprattutto di PFCD, o al contrario, indurre la formazione di artefatti provenienti dalla lisi cellulare (3,4). Quando non è possibile esaminarlo a fresco, il LS può essere conservato a 4°C per 72h oppure a -80°C per un periodo più lungo (fino a 2 mesi) (5).…”

“…It is widely accepted that synovial fluid (SF) analysis should be performed promptly after arthrocentesis to avoid misdiagnosis of borderline leukocyte counts, misinterpretation of artifacts or missing crystals that dissolve during storage [1]. For scientific investigations, in controversies as well as the distribution of SF to other laboratories for secondary opinion, the possibility of storage and delayed examination for crystals may be beneficial [2].…”

“…For scientific investigations, in controversies as well as the distribution of SF to other laboratories for secondary opinion, the possibility of storage and delayed examination for crystals may be beneficial [2]. Several studies assessed the impact of storage conditions and duration on the number and morphology of monosodium urate (MSU) and calcium pyrophosphate dihydrate (CPPD) crystals [1,[3][4][5]. Crystals can be maintained for prolonged periods without morphological changes by freezing [3,5], which requires refrigerators with a certain storage capacity and is not practicable for sample distribution.…”
