Is there a role for the high-risk multidisciplinary team meeting in thoracic surgery?

Abstract: Despite a higher predicted mortality rate by Thoracoscore, HRMDT patients had the same outcome as lower risk non-HRMDT patients. Within the HRMDT cohort, survival in the operated patients was significantly better than that in non-operated patients in the short term. The HRMDT has managed to offer patients a radical treatment option who might have been refused surgery prior to this due to their higher risk profile. We would recommend this forum as a means to further assess and discuss high-risk patients.

“…Similarly, in dealing with patients with a complex disease that requires frequent office visits, hospital admissions, and various home services, the role of an effective social worker appears critical. Furthermore, multidisciplinary meetings have been shown to positively affect patient assessment, management practices, and outcomes across specialties 24–27 . Although data on meeting frequency and outcome measures are unclear, open and streamlined communication between various team members is a critical component of effective team processes and includes structured and regularly scheduled meetings 22 .…”

“…Latterly, endobronchial ultrasound transbronchial needle aspiration, cranial magnetic resonance imaging, transthoracic echocardiogram and exercise testing (either shuttle walk test or cardiopulmonary exercise testing) were also undertaken routinely. A multidisciplinary team assessed each case prior to surgery and all patients were also discussed at specially convened high-risk multidisciplinary team meetings [ 11 ].…”

“…Riskstratification models, such as Thoracoscore for thoracic surgery, are used as aids to augment the decision-making process. However, it is well recognised that Thoracoscore does not predict mortality equally accurately across all patient groups (22), perhaps as a result of changing patient characteristics since its inception, and hence the predictions made by these tools must be considered alongside other components of the pre-operative decision-making process, such as objective functional testing and multidisciplinary team meetings convened specifically to discuss high-risk patients (23). Final decisions regarding treatment should be made on an individual case by case basis.…”
