IUse of Mean platelet Volume as an initial diagnostic marker in evaluation of dengue fever

Abstract: Background: Dengue fever is the most rapidly spreading mosquito-borne viral disease. Four main characteristic manifestations of dengue illness are (i) continuous high fever lasting 2-7 days; (ii) haemorrhagic tendency as shown by a positive tourniquet test, petechiae or epistaxis; (iii) thrombocytopoenia (platelet count <100×109/l); and (iv) evidence of plasma leakage manifested by haemoconcentration (an increase in haematocrit 20% above average for age, sex and population), pleural effusion and ascites, etc. … Show more

“…Increased platelet volume increases platelet adhesion, aggregation and synthesis of thromboxanes which can lead to complications like disseminated intravascular coagulation. [2,5] Prakash GM et al found no correlation between increased PDW and dengue fever which was concurrent with the present study where a statistical significant value was not obtained. Nehara HR et al suggested that high PDW is more sensitive for dengue hemorrhagic fever and more specific for dengue fever.…”

“…Afsar N et al also reported 86.7% of cases in their study to be under 30 years of age. [5,10] Thrombocytopenia indicates a fall in platelet count to <150 x 10 3 mm 3 . Slichter SJ et al suggested that dengue virus induces anti-platelet antibodies that cause vigorous platelet destruction through apoptosis and lysis by complement system.…”

“…Our study showed a significantly higher MPV, PDW, and P-LCR among dengue patients when compared to the control group. Afsar et al [23] also observed a significant increase in MPV values in dengue cases when compared to control group. Contrary to this finding, in a study done by Bashir et al, [24] a decreased MPV and a high PDW were noted in dengue patients when compared to healthy controls.…”

“…11 The tests are simple, inexpensive and non invasive. 10,[12][13][14][15][16] MPV is an indicator of platelet size and is reported as a ratio of Plateletcrit to total platelet count in Hematology Analyser by different methods like Impedence, optical light scatter counting etc. 10,11,16,17 Normal MPV range for our study is 7.5 -10.5 fl 13,16 but differ in other due to many variables.…”
