Ivermectin-Responsive Crusted Scabies in HTLV1 Carrier

Abstract: We report a case of HTLV1 infection revealed by crusted scabies and widespread dermatophytosis in an African woman. HTLV1 infection was not complicated by adult T cell leukemia or myelopathy. Crusted scabies is a marker of HTLV1 infection. The importance of oral ivermectin therapy in crusted scabies is emphasized.

“…In general, the efficacy of ivermectin to ordinary scabies is high with a success rate of 70 and 74% after a single dose of 100 and 200 μ g/kg, respectively. [23][24][25] However, scabies in ATLL patients seems to be resistant to such conventional treatments, 5,7,8 as observed in our case. At present, the mechanism underlying the resistance to conventional therapies remains unknown.…”

“…A single 9-mg oral dose of Ivm (170 lg/Ivm/kg body weight) cleared the skin lesions in a few days. 38 In the study conducted by Dourmishev et al 26 described earlier, 5 immunosuppressed patients were treated with liquid Ivm (1% solution) at a dose of 200 lg/ Ivm/kg body weight weekly for 2 weeks; all were cured 1 week after the second dose of Ivm. These patients were probably included in their subsequent reports.…”
