Jaw Osteomyelitis as a Complication in Osteopetrosis

Abstract: Osteopetrosis is a rare bone dysplasia. The disease is characterized by osteoclast dysfunction, producing diffuse symmetrical increase in skeletal bone density and exhibiting various clinical manifestation because of heterogeneous entity. Among them, jaw osteomyelitis, frequently mandibular osteomyelitis, is an important complication encountered in these patients. In this article, 2 patients diagnosed with osteopetrosis with mandibular osteomyelitis would be presented. We used debridement and decortications, r… Show more

“…Patients are usually asymptomatic with only radiographic signs. 16 This form of OP mimics orthopedic and orofacial symptoms of malignant disease such as nerve compression, macrocephaly, osteomyelitis, reduced arm span and several bone fractures. Ankylosed teeth might be seen in these patients making them susceptible to osteomyelitis of the jaw.…”

“…Bu amaçla önce-likle enfekte alana debridman uygulanır. Debridman sırasında immun yanıtın ve medikal tedavinin etkin olabilmesi için reperfüzyonun sağlanması amacı ile mutlak olarak enflame mandibula periostu uzaklaştırılmalıdır (12). Penisilin, klindamisin, linkomisin, vankomisin, metronidazol, sefalosporinler ve florokinolonlar medikal tedavide tercih edilen antibiyotiklerdir (12).…”

“…[ 8 9 ] Topical phenytoin was used in osteopetrosis patients to enhance bone and soft tissue healing in the mandible. [ 10 ] Induction of fibroblast proliferation, enhancing the formation of granulation tissue, decreasing collagenase activity, promoting the deposition of collagen and other connective tissue components, preventing bacterial contamination, and decreasing wound exudate are some of the phenytoin's mechanisms of action for enhancing the healing process. [ 11 12 ] Applying topical antibacterial agents through their high local therapeutic levels in bony lesions can enhance the healing process of MRONJ lesions.…”
