Aim: In order to clarify how nurses respond to telephone consultations in a Kampo medicine outpatient clinic, a fact-finding survey was conducted on the difficulties involved in handling telephone calls. Methods: The subjects were two outpatient nurses. The contents of consultations that the nurse could not handle alone were extracted from the descriptive data of the telephone calls that each nurse was not able to handle and categorized. Guidelines developed in conjunction with physicians were introduced to clarify and address the causes of the difficulty. One year later, we again administered the telephone response survey so as to compare the results with those of the first survey. Results: Telephone consultations that were difficult for nurses to handle were classified into four categories: 1) consultations about physical disorders, 2) questions about prescribed Kampo medicines, 3) reports and questions based on the patient's own judgment, and 4) questions about treatment and hospitalization. The questions about Kampo medicines related to their "continued administration" and "mix-ups of drugs," and the questions based on patients' self-judgments related to the "reduction of" or "change in" Kampo medicines. One year later, the number of such cases declined to less than half, and no telephone consultations were made within one week after the first visit. Conclusion: As a result of the creation and introduction of guidelines in consultation with physicians, outpatient nurses are now able to answer questions and receive fewer phone calls after patients' first visits.

BackgroundMycophenolic acid (MPA) treatment requires therapeutic drug monitoring to improve the outcome after organ transplantation. The aim of this study was to compare two methods, a particle enhanced turbidimetric inhibition immunoassay (PETINIA) and a reference liquid chromatography tandem mass spectrometry (LC-MS/MS) for determining plasma MPA concentrations from Japanese lung transplant recipients.MethodsPlasma MPA concentrations were determined from 20 Japanese lung transplant recipients using LC-MS/MS and the PETINIA on the Dimension Xpand Plus-HM analyzer.ResultsThe mean MPA concentration measured by PETINIA was significantly higher than that measured by LC-MS/MS (3.26 ± 2.73 μg/mL versus 2.82 ± 2.71 μg/mL, P < 0.0001). The result of the Passing Bablok analysis was a slope of 1.104 (95% confidence interval [CI], 1.036–1.150) and an intercept of 0.229 (95%CI, 0.144–0.315). Bland–Altman analysis revealed PETINIA overestimates plasma MPA concentration by 26.25% and 95%CI from 21.43 to 31.07%.ConclusionThe measurement of MPA by the PETINIA in Japanese lung transplant patients should evaluate the result with attention to positive bias.

Aims : To clarify any difficulties that pharmacists and nurses may have when prescribing Kampo medicines to newly-admitted patients with a survey questionnaire at a local hospital ward. Methods : Questions for pharmacists on the handling of Kampo herbs, any perceived risks in the preparation of Kampo formulae powder extracts/pills, or in explaining Kampo medicines, based on their experiences. Also questions for nurses on the handling of Kampo medicines, as compared to western medicines, and any perceived risks in their administration at their ward. Result : All 7 pharmacists and 14 out of the 16 nurses surveyed completed their questionnaires. The pharmacists pointed out that Kampo preparation takes more time, although none perceived an increased risk with Kampo medicines, as compared to western medicines. Only 1 pharmacist had ever had experience explaining Kampo medicines to patients. The nurses, on the other hand, felt that Kampo treatments were somewhat more difficult to use, and perceived similar risk in their administration to patients.
