Although sexual and reproductive health education and services are provided to young people, current rates of HIV infection and pregnancy are increasing in Nepal, indicating that young people do not always use sexual health services. Health facilities have apparently failed to provide young people with specialized sexual health education and services. This study explored the barriers to using sexual health services, including condom-use among young people in Nepal. Participants from 10 focus groups and 31 in-depth interviews, carried out by a same-sex researcher, reported many socioeconomic, cultural and physical norms that impose barriers to accessing information on sexual health and relevant services. It is concluded that the establishment of youth-friendly service centres in convenient places might help encourage young people to use sexual health services.

Socioeconomic inequalities in contraceptive use among female adolescents in south Asian countries: a decomposition analysis

Background Contraceptive knowledge and use has been an emerging topic of interest in adolescents in Asia. This study quantified the contribution of the socioeconomic determinants of inequality in contraceptive use among currently married female adolescents (15–24) in four south Asian countries: India, Bangladesh, Nepal and Pakistan. Data and methods The data of Demographic Health Survey (DHS) for four South Asian countries, i.e. India (NFHS 2015–16), Nepal (DHS 2016), Bangladesh (DHS 2014) and Pakistan (DHS 2012–2013) has been used for examining the contraceptive use and inherent socioeconomic inequality. After employing logistic regression, concentration curves based on decomposition analysis have been made to analyse the socioeconomic inequality. Results The results reveal that the use of contraception among female adolescents remains low and factors like education, employment, having one or more children, media exposure were positively associated with it. In terms of socioeconomic inequality, a significant amount of variation has been observed across the countries. In India, poor economic status (95.23%), illiteracy (51.29%) and rural residence (23.06%) contributed maximum in explaining the socioeconomic inequality in contraceptive use among female adolescents. For Bangladesh, the largest contributors to inequalities were rural residence (260%), illiteracy (146.67%) while birth order 3 + (− 173.33%) contributed negatively. Illiteracy (50%), poor economic status (47.83%) and rural residence (16.30%) contributed maximum to the inequalities in contraceptive use in Pakistan while birth order 3 + (− 9.78%) contributed negatively. In Nepal, the important operators of inequalities were unemployment (105.26%), birth order 3 + (52.63%) and poor economic status (47.37%), while rural residence contributed negatively (− 63.16%) to inequalities in contraceptive use. Conclusions Using a cross country perspective, this study presents an socioeconomic inequality analysis in contraceptive use and the important factors involved in the same. Since the factors contributing to inequalities in contraceptive use vary across countries, there is a need to imply country-specific initiatives which will look after the special needs of this age-group.

Young people’s sexuality is often stigmatized in South Asian societies, including Nepal, where premarital sex is not accepted socially and culturally. However, the prevalence of early sexual activities among adolescents has increased over time. Given this background, this paper aimed to explore the prevalence of premarital sexual behaviour among the urban high school adolescents. This study utilized a cross-sectional design and collected a total of 609 students of grade 9-12 [ages 10-19] from both community and institutional schools in the Kathmandu metropolis. A validated structured questionnaire was employed and data were analyzed using SPSS with version 24. Univariate, bivariate, and multivariate analyses were performed to interpret the results. The study showed that the mean age of the respondents was 16 years. More than half of adolescents (52.4%) ever watched pornography, amongst whom eight percent did it on a regular basis. Similarly, 14 percent of in-school adolescents ever experienced premarital vaginal sexual intercourse, where the median age for the sexual debut was 16 years. Male respondents tended to be 13 times more likely to have premarital sex compared to the girls. Respondents who were exposed to pornography materials were likely to be exposed by 5.7 times higher than that who were not. Further, 48 percent respondents who ever had sex did not use condoms during their first sexual intercourse, followed by 45 percent who had multiple sex partners.The study concluded that a notable proportion of adolescents was involved in premarital sexual behaviours followed with vaginal sexual intercourse, especially among the boy students. Hence, the results of this study suggest school-based comprehensive sexuality education to minimize the risky sexual behaviour and promote the safer sexual activities among the secondary level school adolescents.
