Konservative Therapie der Gonarthrose

Abstract: Osteoarthritis of the knee is a degenerative joint disease with progressive degradation of articular cartilage and subchondral bone. Symptoms may include joint pain, tenderness, stiffness, locking and joint effusion depending on the stage of the disease. In an effort to delay major surgery, patients with knee osteoarthritis are offered a variety of nonsurgical modalities, such as weight loss, exercise, physiotherapy, bracing, orthoses, nonsteroidal anti-inflammatory drugs (NSAIDs) and intra-articular viscosupp… Show more

“…Treatment options for gon- and coxarthrosis can be divided into conservative therapies (pharmacological and non-pharmacological treatments) and surgical treatments [ 7 11 ]. Surgical options include arthroplasty, osteotomy, resecting arthroscopic procedures and repair of the damaged articular cartilage [ 9 , 11 ]. Endoprosthetic replacement of the affected joint is an effective measure to improve functionality and relieve pain [ 8 ].…”

“…In some patients, treatment cannot be continued due to side effects, while in others, despite the treatments administered, adequate and efficient pain control cannot be obtained [8]. In patients with OA-associated chronic knee pain that does not respond to conservative treatments, and among patients who do not want to undergo operations or find surgery is counter-indicated due to accompanying pathologies, the application of radiofrequency (RF) ablation to the intra-articular and periarticular regions is a novel and effective treatment option reported in the literature.…”

“…Im Rahmen des Behandlungskanons gilt mittlerweile insbesondere die an die individuelle Beschwerdesymptomatik des Patienten adaptierte Bewegungstherapie als evidenzbasiert [9]. Allerdings zeigen die Ergebnisse jün-gerer Reviews, dass sich die induzierten positiven Behandlungsergebnisse ohne weitere therapeutische Begleitung nach 2 bis 6 Monaten signifikant verschlechtern [1, 10,19].…”
