Background: Postgraduate training of medical specialists by the residence system is intimately connected with the functional development of health system in Spain, however, this is a complex phenomenon that involves many people and structures. On the one hand, the resident doctor (MIR) must adapt to a dual role as learner and as health care provider, on the other hand, accredited units for teaching and especially resident"s tutors assume a society responsibility for teaching and training to the future professionals of the health system. This whole process is developed on the basis of specific legal rules, with programs and formative designs that must adapt to the continuous progress of medicine and education in skills, and looking for a complex balance between responsibility for the progressive acquisition of skills by the resident doctor and the obligation of the safety of the patient. Additionally this is a dynamic system, subject to the constraints of pressure care, to the management and optimization of available material and human resources, as well as to personal, team and institutional relationships. Satisfaction in medical education is one of the main indexes of the quality of the process and its evaluation is a legal requirement. Knowing the perception of the main actors (resident doctors and tutors) involved in the obstetrics and gynecology specialty training will allow us to identify those factors affecting the learning process and the satisfaction, in order to develop and implement specific corrective actions. Material and methods: The study has been conducted from the opinion and satisfaction polls about the specialized medical training carried out by the Regional Health Management Unit through the Human Resources General Direction, addressed to the residents and their tutors within Castilla y León autonomic region. It has arranged statistical reports for the period 2010-2013, with the survey´s overall results and detailed by specialty, as well as micro-data from the specialty of Obstetrics and Gynecology provided by the Training Service of the Regional Health Management Unit. Results: The response rate to the satisfaction poll on the Obstetrics and Gynecology specialty training was 73%-75% for the residents and 92% for the tutors (38% and 43.8% respectively, in the year 2003, the first one in which the on-line poll was held). Since the implantation of satisfaction survey in Castilla y León, there has not been a clear improvement on the perception of the resident`s satisfaction. Main causes associated to a detriment in the perception of the Obstetrics and Gynecology resident satisfaction are subsidiaries of resolution at the Health Center level or at the training Unit level (assessment of the resident"s opinion about the job planning, equal treatment among professionals, work overload, management`s commitment to the resident welfare), and some of them should be resolved in compliance with the legislation in force (estimation of progression with the tutor, structured interview, supervision of the degre...
