Fractures and dislocation of trapezium are rare, and diagnosis could be missed. Special radiographic view and a CT scan could be performed to help for the diagnosis. Poor or inadequate treatment of displaced fracture could lead to long-term morbidity. Authors reported an unusual case of vertical trapezium fracture with complete dislocation. Open reduction and internal Wxation allow having an excellent thumb function. A report of this case and the surgical technique are combined with a discussion of literature.
