Laparoscopic Cholecystectomy: Complications and Conversions With the 3-Trocar Technique

Abstract: The study achieves to conclude that the 3-trocar technique can be safely performed, is not technically demanding, and yields in experienced hands a complication rate comparable with the conventional LC. Furthermore, it is less expensive and has a better cosmetic result.

“…We know that the incidence rate of major complications (common bile duct and major vessel injury) following three or four-trocar laparoscopic cholecystectomy is well documented at <1% with an overall complication rate of ≤3%. 40,41 Post-laparoscopic cholecystectomy pain and recovery time is also significantly lower when compared to the alternative open procedure. 2 Whether there is less postoperative pain associated with NOTES or SILS is so far a subjective conclusion and systematic objective assessments of post-procedural pain, as well as procedure-related complication rates, are lacking.…”

“…The overall complication rate of CLC is 3% and common bile duct injury occurs in less than 1% [14]: it is our responsibility to make sure that SUILC does not result in worse outcomes [9]. Our series of 65 patients is one of the largest published to date.…”

“…The surgical technique reported here resembles the three-trocar technique for laparoscopic surgery [45][46][47]. Dissection is supported by the long bended grasper, which gives an additional degree of freedom through rotation of the axis of the grasper.…”
